【摘 要】目的：于產(chǎn)婦中應(yīng)用自由體位分娩，分析該體位分娩方法在提升自然分娩率和降低剖宮產(chǎn)率中所起到的作用。方法：于2014年10月-2016年10月期間，以隨機(jī)數(shù)表法為分組原則，將從我院挑選的60例產(chǎn)婦分為對(duì)照組和觀察組，各組30例，對(duì)照組產(chǎn)婦應(yīng)用傳統(tǒng)體位分娩方式展開分娩，觀察組則采用自由體位分娩，對(duì)兩組產(chǎn)婦的自然分娩率和剖宮產(chǎn)率予以比價(jià)分析。結(jié)果：對(duì)照組產(chǎn)婦的自然分娩率為60.00%，觀察組產(chǎn)婦的自然分娩率為86.67%，觀察組的自然分娩率顯著高于對(duì)照組，統(tǒng)計(jì)學(xué)意義顯著（P<0.05）;對(duì)照組產(chǎn)婦的剖宮產(chǎn)率為40.00%，觀察組產(chǎn)婦的剖宮產(chǎn)率為13.33%，觀察組的剖宮產(chǎn)率顯著低于對(duì)照組，統(tǒng)計(jì)學(xué)意義顯著（P<0.05）;與對(duì)照組比較分析，第一產(chǎn)程、總產(chǎn)程觀察組顯著較短，組間差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論：產(chǎn)婦以自由體位展開分娩的臨床效果顯著，提升了自然分娩率，降低了剖宮產(chǎn)率，同時(shí)縮短了第一產(chǎn)程時(shí)間和總產(chǎn)程時(shí)間，值得在臨床中推廣應(yīng)用。

Abstract Objective： to analyze the effect of the free position delivery method on increasing the rate of natural delivery and decreasing the rate of cesarean section in the puerpera. Methods： in October 2014 - in October 2016， in a random indicator method for grouping principles， 60 cases of maternal from our selected can be divided into control group and observation group， each group 30 cases and control group applied traditional maternal position birth childbirth way， observation group USES the childbirth free position， the natural births and cesarean section rate of two groups of maternal comparison analysis. Results： the rate of natural childbirth in the control group was 60.00%， and that in the observation group was 86.67%. The rate of natural childbirth in the observation group was significantly higher than that in the control group （P < 0.05）. The cesarean section rate of the control group was 40.00%， and that of the observation group was 13.33%. The cesarean section rate of the observation group was significantly lower than that of the control group （P < 0.05）. Compared with the control group， the observation group of the first and total labor stages was significantly shorter， and the difference between the groups was statistically significant （P < 0.05）. Conclusion： the clinical effect of free postpartum delivery is significant， which can increase the rate of natural delivery， reduce the rate of cesarean section， and shorten the first and total labor time.

對(duì)于女性而言，分娩屬于正常的生理過程之一，但該過程中卻會(huì)給女性造成較為嚴(yán)重的生理疼痛，甚至導(dǎo)致產(chǎn)婦產(chǎn)生多種負(fù)面情緒，繼而導(dǎo)致應(yīng)激反應(yīng)，打破產(chǎn)婦的適應(yīng)性與平衡性，最終對(duì)分娩過程產(chǎn)生了影響。換言之，分娩過程中的疼痛會(huì)導(dǎo)致產(chǎn)婦產(chǎn)生恐懼感，繼而導(dǎo)致產(chǎn)婦選擇剖宮產(chǎn)，而在這種情況下，近幾年來剖宮產(chǎn)比例呈現(xiàn)出逐年上升的趨勢(shì)，而剖宮產(chǎn)作為有創(chuàng)分娩方式，往往會(huì)給產(chǎn)婦的身體健康造成嚴(yán)重影響，甚至?xí)?dǎo)致多種并發(fā)癥。為此，降低剖宮產(chǎn)率具有重要意義。過去產(chǎn)婦自然分娩時(shí)，分娩姿勢(shì)主要為仰臥位，但是第一產(chǎn)程開始起仰臥位就會(huì)對(duì)對(duì)產(chǎn)婦的腹部大血管產(chǎn)生擠壓，繼而易造成子宮和胎盤出現(xiàn)供血不足，長(zhǎng)此以往，則會(huì)對(duì)胎兒的健康產(chǎn)生影響，同時(shí)仰臥位在產(chǎn)婦分娩過程中其骨盆也處于狹窄的狀態(tài)，并限制了骨盆的我可動(dòng)性，增加了抬頭下降的阻力，繼而減弱了分娩能力和延長(zhǎng)了產(chǎn)程時(shí)間，因此積極探討符合生理需求的條件具有重大意義，而在本文就著重探究了降低剖宮產(chǎn)的方法，即于2014.10～2016.10期間從我院挑選60例產(chǎn)婦，分析了自由體位分娩對(duì)提高自然分娩率、降低剖宮產(chǎn)率產(chǎn)生的作用。
