摘要：目的? 探討心力衰竭（HF）患者的血清腦鈉肽（BNP）與血清白蛋白（ALB）之間的關(guān)系以及低蛋白血癥對(duì)入選患者近期預(yù)后的影響。方法? 連續(xù)入選承德醫(yī)學(xué)院附屬醫(yī)院本部心臟內(nèi)科2017年6月～2018年8月收治的HF患者共150例，根據(jù)ALB濃度將入選者分為低蛋白血癥組（61例）和對(duì)照組（89例），在出院第30 d進(jìn)行隨訪。結(jié)果? 低蛋白血癥組BNP值（1909.64±1640.90）pg/ml高于對(duì)照組（994.67±808.87） pg/ml（P<0.001），低蛋白血癥組谷丙轉(zhuǎn)氨酶（121.21±57.12）U/L高于對(duì)照組（49.45±7.63）U/L （P=0.014）。ALB濃度與BNP對(duì)數(shù)轉(zhuǎn)換值呈負(fù)相關(guān)（P<0.01）。低蛋白血癥組和對(duì)照組患者30 d內(nèi)HF死亡率分別為6.56%和1.12%（P=0.030），30 d內(nèi)HF再住院率分別為11.48%和3.37%（P=0.025）。結(jié)論? ?ALB濃度與BNP值呈負(fù)相關(guān)，合并低蛋白血癥的HF患者近期預(yù)后不良。

Abstract：Objective? To investigate the relationship between serum brain natriuretic peptide （BNP） and serum albumin （ALB） in patients with heart failure （HF） and the effect of hypoproteinemia on the short-term prognosis of enrolled patients. Methods? A total of 150 patients with HF admitted to the Department of Cardiology， Affiliated Hospital of Chengde Medical College from June 2017 to August 2018 were enrolled. The patients were divided into hypoproteinemia group （61 cases）and The control group （89 cases） was followed up on the 30 d after discharge. Results? The BNP value of the hypoproteinemia group （1909.64±1640.90） pg/ml was higher than that of the control group （994.67±808.87） pg/ml （P<0.001）， and the hypoproteinemia group alanine aminotransferase （121.21±57.12） U/L. It was higher than the control group （49.45±7.63） U/L （P=0.014）. The ALB concentration was negatively correlated with the BNP logarithmic conversion value （P<0.01）. The HF mortality rate was 6.56% and 1.12% （P=0.030） in the hypoproteinemia group and the control group， respectively， and the HF rehospitalization rate was 11.48% and 3.37% （P=0.025） within 30 d. Conclusion? The ALB concentration is negatively correlated with the BNP value. The HF patients with hypoproteinemia have a poor prognosis in the near future.

心力衰竭（heart failure，HF）是一種全球性的流行病[1]，是一系列心臟疾病沿時(shí)間發(fā)展而成的最后階段，隨著我國(guó)人口老齡化加速，HF的發(fā)病率持續(xù)增加，對(duì)人類生命安全構(gòu)成了嚴(yán)重的威脅。流行病學(xué)研究顯示該病發(fā)病率已達(dá)0.9%[2]，猝死發(fā)病率為13%[3]，成為導(dǎo)致現(xiàn)代社會(huì)人群死亡的重要原因之一[4]，且失代償性HF患者接受住院治療后1年死亡率仍可高達(dá)30%[5，6]。因此加強(qiáng)相關(guān)研究，盡早對(duì)HF患者進(jìn)行診斷，及時(shí)進(jìn)行適當(dāng)治療，提高生存率是當(dāng)前極為重要的任務(wù)。多項(xiàng)研究都表明BNP對(duì)于HF的診斷、嚴(yán)重程度判斷和預(yù)后評(píng)估均具有重要的臨床意義，尤其在HF的早期確診、性質(zhì)程度判斷，以及指導(dǎo)臨床治療等方面均表現(xiàn)出明顯的優(yōu)越性[7-9]。臨床研究顯示，ALB是預(yù)測(cè)HF預(yù)后不良的指標(biāo)之一[10-12]。雖然BNP和ALB均與HF有明顯的相關(guān)性，但國(guó)內(nèi)外關(guān)于HF患者BNP與ALB相關(guān)性的相關(guān)研究較少，本研究旨在探討HF患者BNP與ALB的相關(guān)性及ALB與HF患者30 d內(nèi)HF死亡率和再住院率之間的關(guān)系。為臨床醫(yī)生對(duì)HF患者的病情評(píng)估、評(píng)價(jià)預(yù)后提供參考。
