DJ-1蛋白在卵巢癌患者血清中的表達(dá)水平及意義

目的 研究DJ-1蛋白在卵巢癌患者血清中的表達(dá)水平及其意義。方法選取卵巢上皮性腫瘤患者52例為患者組,其中良性卵巢腫瘤9例,交界性卵巢腫瘤5例,卵巢癌38例;非卵巢腫瘤患者20例為對照組,采用雙抗體夾心法測定2組患者血清中DJ-1蛋白的水平。結(jié)果對照組血清中DJ-1蛋白水平明顯低于各種卵巢腫瘤患者(P<0.05)。卵巢癌患者血清DJ-1蛋白水平高于交界性卵巢腫瘤和良性卵巢腫瘤患者,交界性卵巢腫瘤高于良性卵巢腫瘤患者(P<0.05);低分化和中分化卵巢癌患者血清DJ-1水平均高于高分化卵巢癌患者(P<0.05);不同手術(shù)病理分期和不同組織學(xué)分類的卵巢癌患者血清DJ-1水平差別無統(tǒng)計(jì)學(xué)意義(P>0.05)。結(jié)論DJ-1參與了卵巢癌的發(fā)生和發(fā)展,可作為卵巢癌診斷及預(yù)后預(yù)測的血清學(xué)指標(biāo)。

卵巢癌為女性生殖器官三大惡性腫瘤之一,起病隱匿且缺少特異性的早期診斷方法,因此多數(shù)患者就診時已屬臨床晚期,患者5年生存率仍徘徊在20%~30%,死亡率高居?jì)D科生殖系統(tǒng)腫瘤首位[1],已成為嚴(yán)重威脅婦女健康與生命的主要腫瘤。近年隨著基因組學(xué)、蛋白質(zhì)組學(xué)和代謝組學(xué)等技術(shù)的廣泛應(yīng)用,與卵巢癌相關(guān)的新的腫瘤標(biāo)志物不斷被發(fā)現(xiàn),為卵巢癌的診斷、治療提供了更多幫助。許多新的研究表明DJ-I蛋白在多種腫瘤中異常表達(dá),且與腫瘤的發(fā)生、發(fā)展、轉(zhuǎn)移及預(yù)后密切相關(guān)[2-4],但關(guān)于其在卵巢癌中的表達(dá)及意義鮮見相關(guān)報道。本研究擬探討血清DJ-1水平作為卵巢癌生物學(xué)標(biāo)志物的可能性,以期為卵巢癌的診斷、監(jiān)測及預(yù)后判斷提供依據(jù)。
1.1 一般資料 2013年1月—9月在河北大學(xué)附屬醫(yī)院婦科住院的卵巢上皮性腫瘤患者52例為患者組,診斷標(biāo)準(zhǔn)參照文獻(xiàn)[5];其中良性卵巢腫瘤9例,交界性卵巢腫瘤5例,卵巢癌38例;卵巢癌中漿液性腺癌23例,黏液性腺癌10例,子宮內(nèi)膜樣癌2例,透明細(xì)胞癌2例,未分化癌1例?;颊吣挲g35~76歲,平均(50.8±6.2)歲;同期住院的非卵巢腫瘤患者20例為對照組,年齡39~75歲,平均(48.6±5.8)歲,2組年齡差別無統(tǒng)計(jì)學(xué)意義(t=1.372,P>0.05)。
1.2 標(biāo)本采集 所有受試對象抽取空腹肘正中靜脈血4 mL,留置于室溫下30 min,8℃以4 000 r/min離心15 min分離血清,-70℃冰箱保存集中待檢。
1.3 指標(biāo)檢測 采用雙抗體夾心法測定血清中DJ-1蛋白的表達(dá),DJ-1單克隆抗體購自Santa Cruz生物科技公司,兔抗羊生物化素二抗購自武漢博士德生物工程公司,采用WD-2102A型全自動酶標(biāo)儀對DJ-1蛋白水平進(jìn)行檢測。
1.4 統(tǒng)計(jì)學(xué)方法 用SPSS 16.0軟件進(jìn)行統(tǒng)計(jì)學(xué)處理。計(jì)量資料呈正態(tài)分布采用±s表示,比較采用t檢驗(yàn);呈偏態(tài)分布采用M(Q)表示,組間比較采用非參數(shù)秩和檢驗(yàn);以P<0.05為差別有統(tǒng)計(jì)學(xué)意義。
對照組血清中DJ-1蛋白水平明顯低于各種卵巢腫瘤,卵巢癌的DJ-1蛋白水平高于交界性卵巢腫瘤和良性卵巢腫瘤,交界性卵巢腫瘤高于良性卵巢腫瘤(P<0.05);低分化和中分化卵巢癌患者血清DJ-1水平均高于高分化卵巢癌患者(P<0.05);不同手術(shù)病理分期和組織學(xué)分類的卵巢癌患者血清DJ-1蛋白水平差別無統(tǒng)計(jì)學(xué)意義,見表1。
腫瘤生成和發(fā)展是極為復(fù)雜的過程,受許多種因素調(diào)控,而腫瘤標(biāo)志物是腫瘤組織本身生成的化學(xué)類物質(zhì),其存在或量變可反映腫瘤的存在及性質(zhì)[6]。DJ-1基因是Nagakubo等[7]發(fā)現(xiàn)的一種新的絲裂原依賴性癌基因,可與Ras等癌基因起協(xié)同作用。DJ-1基因位于1p36,含有8個外顯子,長度24 kb,其含有570 bp的開放閱讀框架(ORF),編碼1個含189個氨基酸的蛋白[8]。人類的DJ-1蛋白又稱R,與鼠的DJ-1蛋白的氨基酸序列同源性為90%。目前較公認(rèn)的研究發(fā)現(xiàn)DJ-1基因的突變可導(dǎo)致早發(fā)性帕金森病的發(fā)生[9]。Vasseur等[10]發(fā)現(xiàn)DJ-1基因?qū)Φ鞍准っ附閷?dǎo)細(xì)胞形成腫瘤有促進(jìn)作用。Zong 等[11]在對膽管癌的研究中發(fā)現(xiàn)DJ-1蛋白高表達(dá),且可能是膽管癌的獨(dú)立預(yù)后因素。乳腺癌患者中DJ-1蛋白及其抗體較非癌者明顯增多,乳腺癌帶瘤者外周血中可檢測出DJ-1抗體,DJ-1蛋白被認(rèn)為是一種潛在腫瘤抗原與標(biāo)志物,乳腺癌中DJ-1蛋白明顯增高且可作為預(yù)后指標(biāo)[12]。
本研究發(fā)現(xiàn)各種卵巢腫瘤患者血清DJ-1蛋白水平均高于對照組,且卵巢癌患者血清DJ-1蛋白水平高于交界性卵巢腫瘤和良性卵巢腫瘤患者,交界性卵巢腫瘤高于良性卵巢腫瘤患者。Pardo等[13]研究發(fā)現(xiàn)眼葡萄膜惡性黑色素瘤能分泌和表達(dá)DJ-1蛋白,且可在眼葡萄膜惡性黑色素瘤患者的血清中檢測出DJ-1蛋白,提示DJ-1蛋白可能成為一種診斷卵巢癌的潛在血清標(biāo)志物。本研究還發(fā)現(xiàn)不同組織學(xué)分類的卵巢癌患者血清DJ-1蛋白水平差別無統(tǒng)計(jì)學(xué)意義,而低分化和中分化的卵巢癌患者血清DJ-1蛋白水平均高于高分化的卵巢癌患者,提示DJ-1在卵巢癌中不僅參與各種卵巢腫瘤的發(fā)生,還可作為療效監(jiān)測和預(yù)后預(yù)測的指標(biāo)。

不同手術(shù)病理分期的卵巢癌患者血清DJ-1蛋白水平差別無統(tǒng)計(jì)學(xué)意義,其原因可能為本研究樣本量較小,下一步工作將擴(kuò)大樣本量,借助更加精確的分子生物學(xué)技術(shù)和體內(nèi)外實(shí)驗(yàn)來研究DJ-1基因?qū)β殉步M織細(xì)胞的增殖和調(diào)控;同時還可以借助對DJ-1基因進(jìn)行前瞻性研究來探索其對卵巢癌復(fù)發(fā)和預(yù)后的預(yù)測價值。

Expression and Clinic Significance of Serum DJ-1 in Ovarian Cancer

ObjectiveTo investigate serum DJ-1 expression and its significance in ovarian cancer.MethodsThe expression levels of DJ-1 protein were detected by double antibody sandwich method in 52 cases of ovarian tumors(9 cases of benign ovarian tumors,5 cases of borderline ovarian tumors and 38 cases of ovarian cancer,ovarian tumor group and 20 non-ovarian tumors(control group).ResultsThe expression of DJ-1 protein was lower in the control group than that in ovarian tumor group(P<0.05).The expression of serum DJ-1 protein was higher in ovarian cancer than that in borderline ovarian tumors and in benign ovarian tumors.The expression of serum DJ-1 protein was higher in borderline ovarian tumors than that in benign ovarian tumors(P<0.05).The expression of serum DJ-1 protein was higher in poorly differentiated and moderately differentiated ovarian cancer than that in high-grade differentiated ovarian cancer(P<0.05).No significant difference was found in the expression serum of DJ-1 protein between different pathological and different histological types of ovarian cancer(P>0.05).ConclusionDJ-1 might be used as a serum marker in the diagnosis of ovarian cancer.It might be involved in the pathogenesis of ovarian cancer,and could predit the development and prognosis of ovarian cancer.
