摘要：目的 ?對(duì)上海中醫(yī)藥大學(xué)附屬龍華醫(yī)院（以下簡(jiǎn)稱“龍華醫(yī)院”）住院患者華蟾素注射液使用情況進(jìn)行點(diǎn)評(píng)及分析，為提高醫(yī)囑質(zhì)量及促進(jìn)合理用藥提供參考。方法 ?采用龍華醫(yī)院處方分析系統(tǒng)篩選2016年12月1日-2017年12月1日使用華蟾素注射液住院醫(yī)囑，共計(jì)753例患者12 827份醫(yī)囑。對(duì)其適應(yīng)證、用法用量、聯(lián)合用藥及不良反應(yīng)進(jìn)行點(diǎn)評(píng)及分析，判斷其用藥合理性。結(jié)果 ?不合理用藥醫(yī)囑4953份（38.61%），主要問題包括溶媒不適宜、超劑量使用、重復(fù)用藥、藥物相互作用。結(jié)論 ?龍華醫(yī)院使用華蟾素注射液的住院醫(yī)囑存在不合理情況，臨床藥師應(yīng)加強(qiáng)與醫(yī)師交流，規(guī)范醫(yī)囑用藥，更好地為臨床提供藥學(xué)服務(wù)，促進(jìn)合理用藥。

Abstract： Objective?To comment and analyze inpatient prescriptions using Huachansu?Injection in Longhua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine （hereinafter referred to as “Longhua Hospital”）; To provide references for improving the quality of inpatient prescriptions and promoting rational medication. Methods?Longhua Hospital Prescription Analysis System was used to screen the use of Huachansu?Injection in Longhua Hospital from December 1， 2016 to December 1， 2017， involving 12?827 inpatient prescriptions from 753 patients. The indications， usage and dosage， combination medication and adverse reactions were evaluated， and statistical analysis was conducted to judge the rationality of medication. Results?There were 4953 unreasonable medical prescriptions， accounting for 38.61%. The main problems of unreasonable medication prescriptions： inappropriate solvent， overdose， repeated use， and drug interactions. Conclusion?There is an unreasonable situation in the inpatient prescriptions of Longhua Hospital using Huachansu?Injection. Clinical pharmacists should strengthen the communication with doctors， standardize the prescribed medicine， provide better pharmaceutical services for the clinic， and promote the rational medication.
