[摘要] 目的 探討小柴胡湯加減方治療頸淋巴結(jié)結(jié)核的臨床療效。 方法 于2016年1月～2017年12月選擇我院治療的頸淋巴結(jié)結(jié)核60例患者進(jìn)行研究，隨機(jī)分為聯(lián)合組與對(duì)照組各30例。對(duì)照組給予抗結(jié)核治療，聯(lián)合組在此基礎(chǔ)上給予小柴胡湯加減方治療。比較兩組臨床療效及治療前后血沉、淋巴結(jié)最大橫徑。 結(jié)果 兩組治療后淋巴結(jié)最大橫徑均小于治療前，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）;治療后，聯(lián)合組淋巴結(jié)最大橫徑顯著小于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。兩組治療后血沉水平均低于治療前，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）;治療后，聯(lián)合組血沉水平顯著低于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。聯(lián)合組總有效率顯著高于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。 結(jié)論 頸淋巴結(jié)結(jié)核在抗結(jié)核治療基礎(chǔ)上加用小柴胡湯加減方治療能顯著提高臨床療效，促進(jìn)腫大淋巴結(jié)恢復(fù)。

[Abstract] Objective To explore the therapeutic effect of modified Xiaochaihu Decoction on cervical lymph node tuberculosis. Methods 60 patients with cervical lymph node tuberculosis treated in our hospital from January 2016 to December 2017 were selected and randomly divided into the combination group and the control group， with each 30 cases. Patients in the control group were given antituberculosis treatment， while those in the combination group were additionally given modified Xiaochaihu Decoction. The clinical efficacy， the erythrocyte sedimentation rate before and after treatment， and the maximum transverse diameter of the lymph nodes were compared of the two groups. Results The maximum transverse diameter of the lymph nodes after treatment was smaller than that before treatment in both groups （P<0.05）. After treatment， the maximum transverse diameter of the lymph nodes in the combination group was significantly smaller than that of the control group（P<0.05）. The erythrocyte sedimentation rate of the two groups was lower than that before treatment， and the difference was statistically significant （P<0.05）. After treatment， the erythrocyte sedimentation rate in the combination group was significantly lower than that in the control group， and the difference was statistically significant（P<0.05）. The total effective rate of the combination group was significantly higher than that of the control group， and the difference was statistically significant（P<0.05）. Conclusion Modified Xiaochaihu Decoction added to antituberculosis treatment against cervical lymph node tuberculosis can significantly improve clinical efficacy and promote the recovery of enlarged lymph nodes.

頸淋巴結(jié)結(jié)核一般發(fā)生在人體抗病能力低下時(shí)。頸淋巴結(jié)在中醫(yī)上稱為“瘰疬”，中醫(yī)認(rèn)為其病機(jī)主要是肝氣郁結(jié)、脾失健運(yùn)、痰火凝結(jié)、積聚成核[1-2]。小柴胡湯是重要的中醫(yī)方劑，具有和解少陽之功效，主治傷寒少陽病癥。近年來，有報(bào)道將小柴胡湯用于結(jié)核病治療，認(rèn)為其能夠顯著提高抗結(jié)核的療效，抑制炎癥介質(zhì)的釋放，改善預(yù)后[3]。本研究將小柴胡湯加減方用于頸淋巴結(jié)結(jié)核的治療，現(xiàn)報(bào)道如下。

綜上所述，頸淋巴結(jié)結(jié)核在抗結(jié)核治療基礎(chǔ)上加用小柴胡湯加減方治療能顯著提高臨床療效，促進(jìn)腫大淋巴結(jié)恢復(fù)。
