【摘要】 目的 探析老年髖部骨折患者使用股骨近端防旋髓內(nèi)釘（PFNA）內(nèi)固定手術(shù)治療的臨床效果。方法 145例老年髖部骨折患者， 隨機(jī)分為實(shí)驗(yàn)組（73例）和對(duì)比組（72例）。對(duì)比組患者給予股骨近端鎖定鋼板內(nèi)固定手術(shù)治療， 實(shí)驗(yàn)組患者給予PFNA內(nèi)固定手術(shù)治療。比較兩組患者的治療效果、并發(fā)癥發(fā)生情況以及治療相關(guān)指標(biāo)。結(jié)果 實(shí)驗(yàn)組患者治療總有效率為97.26%， 高于對(duì)比組的83.33%， 差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。實(shí)驗(yàn)組患者并發(fā)癥發(fā)生率為5.48%， 明顯低于對(duì)比組的16.67%， 差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。實(shí)驗(yàn)組患者術(shù)中出血量少于對(duì)比組， 手術(shù)時(shí)間、住院時(shí)間短于對(duì)比組， 差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 PFNA內(nèi)固定手術(shù)治療老年髖部骨折患者的效果比較顯著， 能夠有效減少患者術(shù)中出血量， 縮短手術(shù)時(shí)間， 減少并發(fā)癥的發(fā)生， 從而加快患者術(shù)后的恢復(fù)， 縮短住院時(shí)間， 安全性強(qiáng)， 值得在臨床廣泛應(yīng)用與大力推廣。

【Abstract】 Objective? ?To discuss and analyze the clinical effect of proximal femur nail antirotation （PFNA） internal fixation in elderly patients with hip fracture. Methods? ?A total of 145 elderly patients with hip fracture were randomly divided into experimental group （73 cases） and control group （72 cases）. The control group was treated with proximal femur locking plate internal fixation， and the experimental group was treated with PFNA internal fixation. Comparison was made on treatment effect， occurrence of complications and treatment-related indicators between the two groups. Results? ?The total effective rate of treatment in the experimental group was 97.26% in the experimental group， and that was higher than 83.33% in the control group. The difference was statistically significant （P<0.05）. The incidence of complications was 5.48% in the experimental group， and that was 16.67% in the control group. The difference was statistically significant （P<0.05）. The amount of intraoperative hemorrhage in the experimental group was less than that in the control group， and the operation time and hospitalization time were shorter than those in the control group. Their difference was statistically significant （P<0.05）. Conclusion? ?PFNA internal fixation has a remarkable effect for elderly patients with hip fracture. It can effectively reduce the amount of intraoperative hemorrhage， shorten the operation time and reduce the occurrence of complications， thus speeding up the recovery of patients after operation， shortening the hospitalization time with high safety. It is worthy of wide application and vigorous promotion in clinic.

目前， 髖部骨折在老年人群發(fā)病率較高， 主要是因?yàn)槔夏耆艘驗(yàn)槟昙o(jì)逐漸增高， 骨頭強(qiáng)度逐漸下降導(dǎo)致骨質(zhì)疏松， 骨骼越來(lái)越脆， 除此之外， 老年人的髖周肌群慢慢退變， 反應(yīng)速度變慢， 使得老年人一旦出現(xiàn)摔倒、突然扭動(dòng)等情況， 就極易發(fā)生骨折現(xiàn)象， 并且容易引發(fā)各種并發(fā)癥的出現(xiàn) [1， 2]。髖部骨折的出現(xiàn)不僅降低老年人的生活質(zhì)量， 更會(huì)威脅到老年人的生命安全， 因此需要采取積極有效的治療方法對(duì)老年髖部骨折患者及時(shí)進(jìn)行治療[3， 4]。本次研究選擇探析老年髖部骨折患者使用PFNA內(nèi)固定手術(shù)治療的臨床效果， 現(xiàn)報(bào)告如下。
