【摘要】 目的：探討子宮瘢痕妊娠患者經(jīng)過腹腔鏡聯(lián)合宮腔鏡治療的臨床效果。方法：選擇2015年1月-2017年12月江西省婦幼保健院治療的子宮瘢痕妊娠患者60例的資料進(jìn)行回顧性分析。根據(jù)患者的治療方法分為試驗(yàn)組（n=30）和對(duì)照組（n=30）。試驗(yàn)組采用腹腔鏡聯(lián)合宮腔鏡微創(chuàng)手術(shù)治療，對(duì)照組采用甲氨蝶呤化療加清宮治療，分析兩組患者治療效果。結(jié)果：對(duì)照組住院時(shí)間長(zhǎng)于試驗(yàn)組，術(shù)后再次月經(jīng)出現(xiàn)時(shí)間晚于試驗(yàn)組，手術(shù)出血量多于試驗(yàn)組，差異均有統(tǒng)計(jì)學(xué)意義（t=-6.052、-3.621、-5.168，P<0.05）;對(duì)照組術(shù)后出現(xiàn)細(xì)菌感染比例高于試驗(yàn)組，一次性根治患者比例低于試驗(yàn)組，差異均有統(tǒng)計(jì)學(xué)意義（字2=4.706、10.417，P<0.05）。結(jié)論：腹腔鏡聯(lián)合宮腔鏡治療子宮瘢痕妊娠，有住院時(shí)間短、出血量少、術(shù)后感染率低、一次性根治率高、術(shù)后再次月經(jīng)出現(xiàn)時(shí)間快等優(yōu)點(diǎn)。

【Abstract】 Objective：To investigate the clinical effect of laparoscopy combined with hysteroscopy in patients with uterine scar pregnancy.Method：The data of 60 cases of uterine scar pregnancy treated by Jiangxi Maternal and Child Health Hospital from January 2015 to December 2017 were analyzed retrospectively.According to the treatment method of the patients，they were divided into the experimental group（n=30） and the control group（n=30）.The experimental group was treated with laparoscopy combined with hysteroscopic minimally invasive surgery，the control group was treated with Methotrexate plus uterine cure for uterine incision pregnancy.The therapeutic effect of the two groups were analyzed.Result：The hospitalization time in the control group was longer than that in the experimental group，and postoperative menstruation time was later than that in the experimental group，and the amount of bleeding in the operation was more than that in the experimental group，the differences were statistically significant（t=-6.052，-3.621，-5.168，P<0.05）.The proportion of bacterial infection in the control group was higher than that in the experimental group，the proportion of patients who underwent radical cure was lower than that in the experimental group，the differences were statistically significant（字2=4.706，10.417，P<0.05）.Conclusion：Laparoscopy combined with hysteroscopy in the treatment of uterine scar pregnancy has the advantages of short hospital stay，less bleeding，low postoperative infection rate，high one-off radical treatment rate and fast postoperative menstruation.

因此探討診治子宮瘢痕妊娠的安全有效方法，改善患者預(yù)后是目前首先要解決的問題。臨床已有的研究分析出宮腹腔鏡聯(lián)合治療具有明顯的治療優(yōu)勢(shì)：腹腔鏡下觀察可將實(shí)物放大2～4倍，利用氣腹減少出血更可以將盆腔視野清晰地展現(xiàn)出來，以對(duì)盆腔進(jìn)行全面觀察，準(zhǔn)確地評(píng)估盆腔臟器形態(tài)結(jié)構(gòu)、病變制定合理的治療方案[16-17]。Pirtea等[18]綜合目前的文獻(xiàn)表明腹腔鏡方法與異位妊娠切除是最好的選擇。Fu[19]回顧性研究了205例CSP患者，其中189例手術(shù)治療。其中宮腔鏡刮治111例，認(rèn)為宮腔鏡檢查提供了一個(gè)清晰的視野，使我們可以很容易地確定受影響的組織范圍，并清潔妊娠組織。超聲刮除術(shù)是一種相對(duì)簡(jiǎn)單、廉價(jià)的方法。Glenn等[20]綜合了最近的研究表明支持任何消除妊娠和瘢痕的方法，以減少發(fā)病率并促進(jìn)未來的生育能力。腹腔鏡和經(jīng)陰道方法是CSP治療的選擇，盡管需要繼續(xù)研究以確定最佳方法。

本研究中試驗(yàn)組利用宮腹腔鏡聯(lián)合治療子宮瘢痕妊娠，對(duì)照組采用MTX化療加清宮治療子宮瘢痕妊娠。通過比較二組患者的住院時(shí)間、術(shù)中出血、術(shù)后感染、一次性治療成功率均證實(shí)宮腹腔鏡聯(lián)合治療子宮瘢痕妊娠是一種創(chuàng)傷小、安全性高、治療效果確切、依從性好、能最大限度保留生育功能的治療方法，值得臨床進(jìn)一步深入研究探討。
