[摘要] 目的 探討2型糖尿病合并高血壓發(fā)生腦卒中的臨床特點及相關(guān)危險因素。方法 方便選取重慶市渝北區(qū)人民醫(yī)院2010年1月—2015年12月收治的2型糖尿病合并高血壓患者540例，按是否發(fā)生腦卒中分為2型糖尿病合并高血壓合并腦卒中組和2型糖尿病合并高血壓組。比較兩組間年齡、性別、吸煙史，飲酒史，心臟病史、肥胖和各項生化指標(biāo)的差異，分析高血壓分級、糖尿病病程對發(fā)生腦卒中的影響，采用Logistic 回歸分析型糖尿病合并高血壓發(fā)生腦卒中的相關(guān)危險因素。結(jié)果 型糖尿病合并高血壓合并腦卒中組與單純2型糖尿病合并高血壓組比較，發(fā)生腦卒中組年齡（64.00±12.30）歲，較未發(fā)生腦卒中組年齡（59.18±14.69）歲大（t=-2.081， P=0.039<0.05），血糖異常率及血紅蛋白異常率高（P<0.01），糖尿病病程長（P<0.001），處于高血壓2、3級人數(shù)多（P<0.01）；多元Logistic回歸分析結(jié)果顯示年齡（OR=1.089，P=0.007）、糖尿病病程（OR=1.631，P=0.001）、血糖水平（OR=1.951，P=0.003），頸動脈斑塊（OR=5.976，P=0.036）為2型糖尿病合并高血壓發(fā)生腦卒中的危險因素。結(jié)論 2型糖尿病合并高血壓發(fā)生腦卒中與患者血壓、血糖、血脂、糖尿病病程、頸動脈斑塊形成有關(guān)。

Analysis of Clinical Features and Related Risk Factors of Stroke Patients with Type 2 Diabetes and Hypertension

[Abstract] Objective To study the clinical features and related risk factors of stroke patients with type II diabetes and hypertension. Methods 540 cases of patients with type 2 diabetes and hypertension in our hospital from January 2010 to December 2015 were convenient selected and divided into two groups according to whether the patients were with stroke， and the age， gender， smoking history， alcohol history， cardiac disease history， obesity and various biochemical indexes of the two groups were compared， and the effect of hypertension classification and diabetes course on the stroke was analyzed and the related risk factors of stroke patients with type 2 diabetes and hypertension were analyzed by the Logistic regression. Results The age in the type 2 diabetes and hypertension and stroke group was older than that of simple type 2 diabetes and hypertension group， [（64.00±12.30）years old vs（59.18±14.69）years old]， （t=-2.081， P=0.039<0.05）， and the abnormality rates of blood glucose and hemoglobin were higher（P<0.01）， and the diabetes course was longer（P<0.001）， and the cases with second-level and third-level hypertension were most（P<0.01）； and the multi-Logistic regression analysis results showed that the （OR=1.089， P=0.007）， diabetes course（OR=1.631， P=0.001）， blood glucose level （OR=1.951，P=0.003） and carotid plaque （OR=5.976， P=0.036） were the risk factors of stroke patients with type 2 diabetes and hypertension. Conclusion The patients with type 2 diabetes and hypertension are related to the blood hypertension， blood glucose， blood liquid， diabetes course and carotid plaque formation.endprint
