血清幾丁質(zhì)酶3樣蛋白1與糖尿病足病的相關(guān)性探討

目的 探討糖尿病足病患者血清幾丁質(zhì)酶3樣蛋白1(chitinase-3-like-1 protein,YKL-40)水平與糖尿病足病的關(guān)系。方法 選取我院內(nèi)分泌科住院的2型糖尿病患者121例作為研究對象,將診斷為糖尿病足病的患者作為病例組,2型糖尿病患者作為T2DM組,同時選取健康體檢者作為對照組。應(yīng)用ELISA檢測所有受試對象的血清YKL-40水平。結(jié)果 病例組和T2DM組的YKL-40顯著高于對照組,病例組的YKL-40顯著高于和T2DM組,差異均有統(tǒng)計學(xué)意義(P<0.05);YKL-40曲線下面積為0.820,cut-off值為73.87 ng/mL,約等指數(shù)為50.08%,靈敏度為67.4%,特異度為82.7%。結(jié)論 YKL-40在糖尿病足病中明顯升高,在糖尿病足病中發(fā)揮重要作用。血清YKL-40可作為早期檢測糖尿病足病的指標(biāo),對糖尿病足病具有一定的診斷價值。
YKL-40;糖尿??;糖尿病足病
糖尿病足?。―iabetic Foot,DF)是指糖尿?。―iabetic Mellitus,DM)患者下肢發(fā)生的血管和神經(jīng)異常,累及下肢皮膚、骨組織等的感染、潰瘍和壞疽[1]。DF的病變程度較重,易導(dǎo)致患者發(fā)生截肢甚至死亡。目前,DF患者的平均年齡為60歲[2],且患病率呈上升趨勢。DF所致的截肢占非創(chuàng)傷性截肢的33%[3]。然而,國內(nèi)外對于YKL-40與DF的關(guān)系研究較少,本文通過分析DF患者血清YKL-40水平變化,探討血清YKL-40水平與DF的關(guān)系和其診斷價值。
1.1 一般資料
選取我院內(nèi)分泌科住院的2型糖尿病患者121例作為研究對象,診斷為糖尿病足病的患者69例作為病例組,2型糖尿病患者52例作為T2DM組,糖尿病的診斷標(biāo)準(zhǔn)按照1999年WHO關(guān)于糖尿病的診斷標(biāo)準(zhǔn)。隨機選取正常體檢者52例作為對照組。
1.2 方法
所有受試對象于清晨空腹時采血3 mL,送入我院檢驗科離心機以3000 r/min離心,10 min后提取上層血清,置于-80℃冰箱中保存,采用ELISA測定血清YKL-40濃度。
1.3 統(tǒng)計學(xué)方法
2.1 各組間年齡、性別、血壓、TC、TG、HDL和LDL差異無統(tǒng)計學(xué)意義(P>0.05),具有可比性。病例組的Hb低于T2DM組和對照組,差異有統(tǒng)計學(xué)意義(P<0.05);病例組和T2DM組的HbA1c、FPG高于對照組,差異有統(tǒng)計學(xué)意義(P<0.05);病例組和T2DM組的YKL-40水平顯著高于對照組,病例組的YKL-40水平顯著高于T2DM組,差異均有統(tǒng)計學(xué)意義(P<0.05);說明血清YKL-40是DF的影響因素。見表1,表2。

2.2 DF患者YKL-40水平ROC曲線分析
YKL-40曲線下面積為0.820,cut-off值為73.87 ng/mL,約等指數(shù)為50.08%,靈敏度為67.4%,特異度為82.7%,陽性似然比3.89%,陰性似然比為0.39%,陽性預(yù)測值為83.8%,陰性預(yù)測值為65.6%,見圖1。

YKL-40是新近發(fā)現(xiàn)的一種血清炎癥和內(nèi)皮損傷標(biāo)記物。YKL-40的本質(zhì)是一種糖蛋白,由肝素、幾丁質(zhì)和膠原蛋白結(jié)合而成的[4]。許多炎性疾病均可分泌YKL-40,如骨關(guān)節(jié)炎[5]、T2DM[6]、冠心病[7]等。動脈粥樣硬化(AS)是DF患者血管病變的病變基礎(chǔ)[8],而AS是一種慢性炎癥性疾病,與炎癥反應(yīng)密切相關(guān)。YKL-40作為一種炎癥因子,參與體內(nèi)的急慢性炎癥反應(yīng)。因此YKL-40是連接AS和DF血管病變的橋梁。一些研究表明[9]YKL-40參與了糖尿病血管并發(fā)癥的發(fā)生,且能反應(yīng)血管AS的程度,這為早期識別DF提供了線索,在臨床上具有比較大的參考價值。一項對我國臺灣人群的研究[10]結(jié)果指出,YKL-40與糖尿病下肢血管病變的發(fā)生密切相關(guān)。國外有研究發(fā)現(xiàn)[9],位于AS斑塊內(nèi)的巨噬細(xì)胞可以分泌YKL-40,特別是處于病變較深部位的巨噬細(xì)胞亞群中有大量的YKL-40mRNA分泌表達。研究者們建立的旨在研究AS的仿真模型中,發(fā)現(xiàn)YKL-40的mRNA的大量表達[11]。研究表明[12],AS患者血清YKL-40水平顯著高于健康人群,且YKL-40與AS的嚴(yán)重程度相關(guān)。本試驗結(jié)果顯示,病例組的血清YKL-40水平顯著高于T2DM組和對照組,說明血清YKL-40是糖尿病足病的危險因素。通過ROC對YKL-40診斷DF的分析顯示,曲線下面積為0.820,以173.87 ng/mL為cutoff值判斷其診斷靈敏度為67.4%,特異度為82.7%,表明血清YKL-40對糖尿病足病具有一定的診斷價值。因此,血清YKL-40對DF的診斷具有一定的臨床價值,有可能成為糖尿病足病早期預(yù)測指標(biāo),為預(yù)防和治療糖尿病足病提供新的靶點。

Correlation between serum chitinase-3-like-1 protein and diabetic foot

Objective To explore the relationship between diabetic foot and serum YKL-40 level。Methods Selected 121 patients with type 2 diabetes, in which the diagnosis of patients with diabetic foot as case group,type 2 diabetes as T2DM group And selected healthy adults as control group.Serum YKL-40 were detected by ELISA.Results Serum YKL-40 in case group and T2DM group were higer than the control group,Serum YKL-40 in case group was higer than the T2DM group, and the difference had statistical significance(P<0.05);Area under the curve of YKL-40 was 0.820,the cut-off point of YKL-40 was 73.87ng/ml, base on this the youden index was 50.08%,the sensitivity was 67.4%,the specificity was 82.7%.Conclusion Serum YKL-40 in diabetic foot is significantly increased, which plays an important role in diabetic foot. Serum YKL-40 can be used as a predictor of early diabetic foot, and it has a certain diagnostic value for diabetic foot.
