應(yīng)用鎖骨鉤鋼板治療鎖骨遠(yuǎn)端骨折臨床體會(huì)

目的 探討采用鎖骨鉤鋼板治療鎖骨遠(yuǎn)端骨折的療效及并發(fā)癥發(fā)生原因。方法 回顧分析采用鎖骨鉤鋼板治療39例鎖骨遠(yuǎn)端骨折患者資料。采用美國肩肘外科醫(yī)師協(xié)會(huì)(American shoulder and elbow surgeons,ASES)評分評價(jià)術(shù)后肩關(guān)節(jié)功能。結(jié)果 39例患者均獲得隨訪,隨訪時(shí)間為9~15個(gè)月,平均11.6個(gè)月,所有患者均于術(shù)后6個(gè)月內(nèi)骨折愈合,出現(xiàn)肩峰下骨侵蝕37例,肩峰下撞擊癥3例,螺釘松動(dòng)2例。骨折愈合時(shí)行ASES評分為81~91分,平均86.8分;骨折愈合后1~6個(gè)月取出鋼板,鋼板取出3個(gè)月后行ASES評分為86~95分,平均92.1分。結(jié)論 鎖骨鉤鋼板治療鎖骨遠(yuǎn)端骨折并發(fā)癥較高,但鋼板去除后肩關(guān)節(jié)功能恢復(fù)良好,其操作簡便,適宜基層醫(yī)院使用。

鎖骨骨折占成人骨折的2.6%,其中約10%為遠(yuǎn)端骨折[1]。鎖骨遠(yuǎn)端的骨折端通常很小,以往常采用張力帶鋼絲、螺釘或克氏針固定骨折[2],但手術(shù)后早期不易進(jìn)行功能鍛煉,且可發(fā)生退釘、復(fù)位喪失和肩鎖關(guān)節(jié)炎等并發(fā)癥,固定效果不佳[3-4]。近年來,由于操作簡便、療效確切,鎖骨鉤鋼板被廣泛用于治療鎖骨遠(yuǎn)端骨折[5],該方法提高了骨折愈合率,但其在應(yīng)用過程中出現(xiàn)諸如肩峰下骨侵蝕,術(shù)后肩關(guān)節(jié)疼痛、活動(dòng)受限,鎖骨鉤鋼板周圍骨折等并發(fā)癥的報(bào)道也越來越多[6-8]。為此,本研究回顧近年來濟(jì)源二院應(yīng)用鎖骨鉤鋼板治療鎖骨遠(yuǎn)端骨折39例的臨床資料,分析鎖骨鉤鋼板的作用原理及使用技巧,以期對臨床使用有所幫助。
1.1 一般資料 回顧性分析濟(jì)源市第二人民醫(yī)院2008年5月~2012年5月采用鎖骨鉤鋼板治療39例鎖骨遠(yuǎn)端骨折患者的完整資料,其中男29例,女10例;年齡21~63歲,平均38.9歲;左側(cè)16例,右側(cè)23例;按Neer分型[9]:I型2例,II型33例,III型4例。損傷機(jī)制:車禍傷16例,運(yùn)動(dòng)傷10例,墜落傷13例,均為閉合性骨折。傷后至接受手術(shù)時(shí)間24~96h,平均48h。
1.2 手術(shù)方法 頸叢或全身麻醉后,患肩墊高10°~20°,鎖骨遠(yuǎn)端橫行切口,顯露骨折端,適當(dāng)清除軟組織,根據(jù)骨折粉碎情況對骨折塊輔以螺釘固定,復(fù)位滿意后置入鎖骨鉤鋼板,置入時(shí)要注意鋼板不過度下壓鎖骨,可根據(jù)肩峰形態(tài)預(yù)彎鋼板鉤,將鎖骨鉤鋼板緊貼鎖骨后方插入肩峰下方。
1.3 術(shù)后處理 術(shù)后常規(guī)三角巾懸吊患肢。麻醉消退后即行握拳、伸指、前臂旋轉(zhuǎn)及肘關(guān)節(jié)屈伸等功能活動(dòng);傷口疼痛緩解后開始鐘擺樣功能鍛煉;術(shù)后3周可行抬舉活動(dòng),術(shù)后6周逐漸恢復(fù)日?;顒?dòng);在鋼板固定期間避免患肩負(fù)重,上舉活動(dòng)時(shí)幅度不宜超過90°。
1.4 療效評定標(biāo)準(zhǔn) 采用美國肩肘外科醫(yī)師協(xié)會(huì)(American shoulder and elbow surgeons,ASES)標(biāo)準(zhǔn)[10],評價(jià)術(shù)后肩關(guān)節(jié)功能,包括疼痛和活動(dòng)功能2部分,滿分100分,分?jǐn)?shù)越高表示肩關(guān)節(jié)功能越好。
手術(shù)時(shí)間40~50min,平均46min;術(shù)中出血量約40~100mL。該組患者均獲得完整隨訪,隨訪時(shí)間9~15個(gè)月,平均11.6個(gè)月,39例患者均于術(shù)后6個(gè)月內(nèi)骨折愈合,骨折愈合時(shí)行ASES評分為81~91分,平均86.8分。骨折愈合后1~6個(gè)月取出鋼板,鋼板取出3個(gè)月后行ASES評分為86~95分,平均92.1分。
并發(fā)癥:肩峰下骨侵蝕為常見的并發(fā)癥,該組患者出現(xiàn)37例,發(fā)生率為94.8%,該并發(fā)癥的發(fā)生與鉤鋼板自身作用特點(diǎn)有關(guān),臨床癥狀輕微,表現(xiàn)為肩部偶有不適,嚴(yán)重程度與術(shù)后患肩功能無關(guān)。肩峰下撞擊癥3例,發(fā)生率為7.7%,表現(xiàn)為術(shù)后患肩疼痛,上舉受限,取出鋼板1~3個(gè)月后癥狀減輕或消失。2例老年患者術(shù)后1個(gè)月復(fù)查時(shí)發(fā)現(xiàn)鉤鋼板的最內(nèi)側(cè)端螺釘松動(dòng),給予保守治療至骨折愈合鋼板取出,肩關(guān)節(jié)功能恢復(fù)良好。本組患者無鎖骨鉤鋼板周圍骨折、鉤尖穿出肩峰、鉤鋼板斷裂及鎖骨遠(yuǎn)端骨溶解等并發(fā)癥發(fā)生。
肩峰下骨侵蝕的形成原因與鎖骨鉤鋼板的作用特點(diǎn)密切相關(guān),鎖骨鉤鋼板利用杠桿作用的原理通過鉤尖上抬肩峰時(shí)產(chǎn)生的對抗性反作用力來下壓鎖骨達(dá)到對移位的鎖骨遠(yuǎn)端骨折復(fù)位和固定的目的[11],與鉤尖接觸的肩峰骨面是應(yīng)力的集中點(diǎn),骨組軹對壓應(yīng)力的反應(yīng)導(dǎo)致骨吸收,造成肩峰下骨侵蝕的發(fā)生。因此,為了減少侵蝕程度,應(yīng)降低鉤尖上抬肩峰的力量,如在手術(shù)時(shí)選擇合適深度的鎖骨鋼板并適當(dāng)折彎鉤鋼板來避免過度上抬肩峰。在鉤鋼板留存期間應(yīng)避免患者上舉幅度超過90°,以減少鉤尖對肩峰骨面的進(jìn)一步磨損。報(bào)道指出,在肩關(guān)節(jié)上舉90°時(shí),相對于肩峰,鎖骨會(huì)有5°左右后旋和6°左右上移;而當(dāng)上舉加大至150°時(shí),鎖骨后旋范圍加大至27°,上移范圍加大至21°[12],因此做好患者控制上舉度數(shù)的宣教工作非常重要。有鑒于鉤鋼板的肩峰下骨侵蝕是其產(chǎn)品特點(diǎn)所致并發(fā)癥,因此為避免肩峰骨折的發(fā)生,對年輕及老年患者使用鎖骨鉤鋼板應(yīng)慎重。
使用鎖骨鉤鋼板的另一常見并發(fā)癥是表現(xiàn)為疼痛和上舉受限的肩峰下撞擊癥。研究發(fā)現(xiàn),肩峰下間隙減少時(shí)易發(fā)生肩峰下撞擊癥[13],插入鉤鋼板會(huì)導(dǎo)致肩峰下間隙狹窄,損傷肩峰下滑囊引起滑囊炎,同時(shí)鉤鋼板的底部與岡上肌接觸刺激引起岡上肌炎也是患肩疼痛和上舉困難的原因。應(yīng)根據(jù)術(shù)前X線片確定鎖骨鉤鋼板的型號(hào),盡量選用鉤鈍、小、薄且與肩峰相吻合的鎖骨鉤鋼板,手術(shù)時(shí)鉤端應(yīng)盡量置于肩峰的下方、靠后且較硬的肩峰骨質(zhì)下面,緊貼鎖骨后緣骨膜下插入,避免損傷肩袖。對于肩峰下間隙狹窄患者應(yīng)避免使用鎖骨鉤鋼板治療,發(fā)生肩峰下撞擊癥者一般鋼板取出后癥狀減輕或消失。
2例螺釘松動(dòng)者為老年患者,因疏松的骨質(zhì)不能給螺釘提供很好的把持所致。本組患者在選擇治療方案時(shí)年齡選取適當(dāng),無年輕及高齡患者,術(shù)后功能鍛煉指導(dǎo)跟進(jìn)及時(shí),無鋼板斷裂、周圍骨折、鉤尖穿出等并發(fā)癥發(fā)生??傊?鎖骨鉤鋼板操作簡便,學(xué)習(xí)曲線短,便于基層醫(yī)院開展,不失為治療鎖骨遠(yuǎn)端骨折的有效方法。

Objective To evaluate the therapeutic effect and complications of clavicular hook plate in treatment of distal clavicle fracture causes. Methods A retrospective analysis of clavicular hook plate in the treatment of 39 cases of distal clavicle fracture patient data. By the United States association of shoulder and elbow surgeons (american shoulder and elbow surgeons, ASES) of shoulder joint function after operation evaluation. Results 39 patients were followed up, followed up for 9-15 months, average 11.6 months, all patients in 6 months after operation of fracture healing, subacromial bone erosion in 37 cases, 3 patients with subacromial impingement syndrome, screw loosening in 2 cases. Healing by ASES score 81-91 fractures, an average of 86.8 points; The healing after 1-6 months out of steel plate fracture, plate removed 3 months after the ASES score of 86 to 95 points, an average of 92.1 points. Conclusion Complications were higher in the distal clavicle clavicular hook plate in treatment of fracture of steel plate, but after the removal of shoulder joint function recovered well, and has the advantages of simple operation, suitable for primary hospital use.
