        摘要" 肌鈣蛋白是存在于心肌表面的一種特殊蛋白,是心肌細(xì)胞特有的組成成分,慢性牙周炎是以菌斑微生物為始動因子的慢性感染性疾病。在牙周炎發(fā)展過程中,牙周袋中的牙周致病菌及其相關(guān)因子增多,從而誘導(dǎo)Ca2通道打開,Ca2進(jìn)入內(nèi)皮細(xì)胞和心肌細(xì)胞,造成心肌細(xì)胞損傷,肌鈣蛋白復(fù)合物水解,使血清肌鈣蛋白水平升高,肌鈣蛋白可誘導(dǎo)牙周黏附分子等表達(dá),使炎性細(xì)胞黏附,最終導(dǎo)致牙周炎進(jìn)一步發(fā)展。綜述兩者的關(guān)系及相關(guān)機(jī)制。

        慢性牙周炎是以菌斑微生物為始動因子的慢性感染性疾病,可使牙周支持組織發(fā)炎、漸進(jìn)性附著喪失、牙槽骨吸收,最終導(dǎo)致牙齒喪失1-2。牙周炎是口腔科的常見疾病,約占牙周疾病的95%,根據(jù)第四次全國口腔健康流行病學(xué)調(diào)查顯示,我國35歲以上人群中,超過90%的成年人存在不同程度的牙周疾病,且慢性牙周炎也是我國成年人失牙的首要病因3-4。慢性牙周炎持續(xù)增長,引發(fā)嚴(yán)重的醫(yī)療保健問題5。除口腔中的臨床表現(xiàn)外,慢性牙周炎與全身系統(tǒng)性疾病密切相關(guān),包括心血管疾病、糖尿病、慢性阻塞性肺疾病、類風(fēng)濕性關(guān)節(jié)炎、阿爾茨海默病及胰腺癌等疾病的發(fā)生發(fā)展密切相關(guān)6-8。

        肌鈣蛋白是骨骼肌和心肌細(xì)絲的組成部分,其由3個亞基組成——肌鈣蛋白I、肌鈣蛋白T和肌鈣蛋白C,其在肌肉活動中發(fā)揮著重要的作用,可將細(xì)胞內(nèi)Ca2濃度與收縮聯(lián)系起來9。在心肌受到損傷時,可被釋放到循環(huán)中10。在正常情況下血清肌鈣白濃度在0.04 ng/mL以內(nèi),若血清水平升高需警惕心肌是否存在損傷。肌鈣蛋白水平升高可促進(jìn)冠狀動脈鈣化,從而導(dǎo)致心肌損傷11-12。肌鈣蛋白升高的常見原因包括快速性心律失常、心力衰竭、高血壓急癥、危重病、心肌炎、全身性炎癥、應(yīng)激性心肌病和心臟瓣膜病等13。通過降壓可降低肌鈣蛋白水平,血清超敏肌鈣蛋白濃度升高可能與炎癥反應(yīng)有關(guān)。綜述超敏肌鈣蛋白和牙周炎的關(guān)系及其發(fā)生機(jī)制。

        1 牙周炎和超敏肌鈣蛋白的相關(guān)性

        有研究分析人群血清超敏肌鈣蛋白水平,結(jié)果顯示,患牙周炎人群血清超敏肌鈣蛋白濃度高于牙周健康人群14。橫斷面研究發(fā)現(xiàn),血清超敏肌鈣蛋白T(hs-cTnT)濃度升高與牙周病有關(guān)15。有研究比較了伴牙周炎的冠心病病人與單純冠心病病人的血清超敏肌鈣蛋白的濃度,結(jié)果發(fā)現(xiàn),伴牙周炎的冠心病病人血清超敏肌鈣蛋白水平高于單純冠心病病人,揭示了牙周炎與血清超敏肌鈣蛋白可能存在一定的相關(guān)性16

        2 牙周狀態(tài)與肌鈣蛋白的相關(guān)性

        心肌梗死與牙周炎嚴(yán)重程度的相關(guān)研究分析了115例心肌梗死病人的血清超敏肌鈣蛋白水平和牙周指數(shù),結(jié)果發(fā)現(xiàn),血清超敏肌鈣蛋白水平與牙周指數(shù)、牙周炎嚴(yán)重程度呈正相關(guān)17。有研究將病人分為吸煙組牙周炎病人、非吸煙組牙周炎病人和牙周健康受試者,測定牙周指數(shù)、出血指數(shù)等牙周指標(biāo)和血清肌鈣蛋白水平,結(jié)果顯示,肌鈣蛋白水平與心肌梗死病人牙周炎嚴(yán)重程度及牙周狀態(tài)呈正相關(guān)18。

        3 牙周炎與肌鈣蛋白的相關(guān)機(jī)制

        3.1 牙周炎與肌鈣蛋白水平升高的機(jī)制

        3.1.1 牙齦卟啉單胞菌與肌鈣蛋白

        牙齦卟啉單胞菌是一種革蘭陰性口腔厭氧菌,參與牙周炎的發(fā)病機(jī)制,其不斷增殖到一定數(shù)量,且對牙周有高度的破壞性。研究表明,血清肌鈣蛋白水平升高與牙齦卟啉單胞菌及其組成成分、分泌物對心肌細(xì)胞的直接細(xì)胞毒性作用有關(guān)。鈣螯合蛋白(由CASQ2編碼)是一種高容量Ca2-結(jié)合蛋白,位于肌質(zhì)網(wǎng)中,是主要的Ca2釋放通道,可自發(fā)打開19-20。肌鈣蛋白的突變,包括肌鈣蛋白I-3和肌球蛋白輕鏈-2(分別由TNNI3和MYL2編碼),通過Ca2參與電生理異常21-22。有研究顯示,Pg毒力因子降低了心肌細(xì)胞中的場點(diǎn)位持續(xù)時間,阻斷了Ca2通道,使得相關(guān)基因表達(dá)下調(diào),Ca2進(jìn)入心肌細(xì)胞,從而導(dǎo)致肌鈣蛋白復(fù)合物水解,更多的肌鈣蛋白釋放入血,血清肌鈣蛋白水平升高23。

        3.1.2 脂多糖與肌鈣蛋白

        脂多糖由脂質(zhì)和多糖構(gòu)成,是革蘭陰性菌細(xì)胞壁的主要成分之一,也是誘發(fā)炎癥反應(yīng)的重要物質(zhì)。脂多糖可促進(jìn)牙周韌帶細(xì)胞(PDLCs)中炎性細(xì)胞因子釋放24,引發(fā)炎癥反應(yīng),生成M1巨噬細(xì)胞25-26,M1巨噬細(xì)胞廣泛浸潤并導(dǎo)致牙周組織嚴(yán)重破壞及骨吸收27。

        脂多糖可誘導(dǎo)核轉(zhuǎn)錄因子(NF)-κB信號通路激活,在牙周炎進(jìn)展過程中促進(jìn)巨噬細(xì)胞產(chǎn)生M1相關(guān)細(xì)胞因子,如白介素(IL)-6、腫瘤壞死因子(TNF)-α、IL-1β和一氧化氮(NO)28,從而進(jìn)一步導(dǎo)致牙周破壞,脂多糖促進(jìn)了循環(huán)中促炎細(xì)胞因子表達(dá),引發(fā)巨噬細(xì)胞增多,并抑制成纖維細(xì)胞活化和瘢痕形成29,觸發(fā)的細(xì)胞內(nèi)Ca2釋放,之后鈣蛋白酶活化,加速肌鈣蛋白復(fù)合物水解,肌鈣蛋白釋放到循環(huán)中,導(dǎo)致血清肌鈣蛋白水平升高。

        3.1.3 唾液皮質(zhì)醇與肌鈣蛋白

        皮質(zhì)醇是由腎上腺分泌產(chǎn)生的壓力激素,有研究發(fā)現(xiàn),唾液皮質(zhì)醇水平與牙周炎之間及唾液皮質(zhì)醇水平與壓力之間存在關(guān)系;皮質(zhì)醇水平與牙周袋深度呈正相關(guān),且皮質(zhì)醇可調(diào)節(jié)牙周袋內(nèi)某些牙周病原體,特別是紅色復(fù)合體的牙周病原體。一項研究表明,皮質(zhì)醇通過對壓力反應(yīng)可能間接導(dǎo)致肌鈣蛋白T水平增加30。氧化應(yīng)激可能在皮質(zhì)醇誘導(dǎo)的肌鈣蛋白釋放過程中發(fā)揮作用31-32。血清肌鈣蛋白水平增加與心肌丙二醛濃度增加相對應(yīng),丙二醛是自由基脂質(zhì)過氧化的標(biāo)志物33。皮質(zhì)醇通過調(diào)節(jié)離子通道增加肌鈣蛋白水平。皮質(zhì)醇對壓力的反應(yīng)通過增強(qiáng)腎上腺素能信號傳導(dǎo)誘導(dǎo)肌鈣蛋白水平增加。皮質(zhì)類固醇可增強(qiáng)腎上腺素能信號傳導(dǎo),引起肌肉收縮和心肌細(xì)胞肥大,增加心肌細(xì)胞鹽皮質(zhì)激素和糖皮質(zhì)激素受體的功能表達(dá)34。盡管應(yīng)激反應(yīng)可能與肌鈣蛋白呈正相關(guān),但肌鈣蛋白直接或間接調(diào)節(jié)糖皮質(zhì)激素的證據(jù)較少。肌鈣蛋白基因含有轉(zhuǎn)錄因子結(jié)合位點(diǎn),可結(jié)合活化的糖皮質(zhì)激素受體35

        3.2 肌鈣蛋白介導(dǎo)牙周炎的相關(guān)機(jī)制

        3.2.1 轉(zhuǎn)化生長因子β(transforming growth factor-β,TGF-β)

        TGF-β是一類結(jié)構(gòu)保守的多功能細(xì)胞因子,廣泛影響細(xì)胞的增殖、分化、胞外基質(zhì)中產(chǎn)物的生成等一系列過程,在胚胎發(fā)生、造血調(diào)控、免疫調(diào)節(jié)、炎癥反應(yīng)等方面發(fā)揮著重要作用。牙周炎的嚴(yán)重程度與上皮-間充質(zhì)轉(zhuǎn)化(EMT)過程標(biāo)志物(TGF-β和vimentin)的表達(dá)存在相關(guān)性,這種相關(guān)性表明EMT在牙周病的發(fā)病機(jī)制和預(yù)后中發(fā)揮著重要作用36。體外研究表明,肌鈣蛋白可激活TGF-β信號傳導(dǎo)的基因,顯著下調(diào)TGF-β水平,并驗證了肌鈣蛋白與TGF-β信號之間的調(diào)節(jié),抑制了TGF-β表達(dá),從而阻礙了牙周組織的再生及恢復(fù)37。

        3.2.2 E-鈣黏蛋白

        E-鈣黏蛋白是一種鈣離子依賴性跨膜蛋白,可調(diào)節(jié)細(xì)胞排列、定位和運(yùn)動以響應(yīng)細(xì)胞外信號傳導(dǎo)38。一項研究表明,牙周炎中的E-鈣黏蛋白可能增加,E-鈣黏蛋白誘導(dǎo)炎癥細(xì)胞積聚,破壞牙齦上皮屏障,從而加重牙周炎39。體外研究顯示,肌鈣蛋白誘導(dǎo)E-鈣黏蛋白表達(dá)增加,促進(jìn)白細(xì)胞積聚,從而造成牙周破壞40-41

        3.2.3 Toll樣受體4(Toll-like receptors 4,TLR4)

        TLR4是由TLR4基因編碼,并表達(dá)于單核細(xì)胞、巨噬細(xì)胞在內(nèi)的多種組織細(xì)胞中,通過NF-κB或c-Jun氨基末端激酶(JNK)/應(yīng)激激活的蛋白酶(SAPK)途徑啟動細(xì)胞內(nèi)的信號轉(zhuǎn)導(dǎo)。TLR4可調(diào)節(jié)體內(nèi)炎癥反應(yīng),從而增加促炎細(xì)胞因子釋放,加劇炎癥反應(yīng)。

        3.2.4 IL

        IL是指在白細(xì)胞或免疫細(xì)胞之間相互作用的淋巴因子,白細(xì)胞介素在傳遞信息、激活與調(diào)節(jié)免疫細(xì)胞、介導(dǎo)T細(xì)胞和B細(xì)胞活化、增殖與分化,在炎癥反應(yīng)中發(fā)揮著重要作用。相關(guān)研究表明,IL增加引發(fā)一系列牙周炎癥反應(yīng),并促進(jìn)牙槽骨吸收42-43。有研究表明,肌鈣蛋白通過激活低氧誘導(dǎo)因子-1(HIF-1)與TLR信號通路,導(dǎo)致IL-18R表達(dá)增強(qiáng),并促進(jìn)炎癥細(xì)胞因子IL-1β和IL-18分泌,引發(fā)宿主炎癥反應(yīng),破壞牙周組織,加重牙周炎44。

        4 外界因素對肌鈣蛋白與牙周炎相關(guān)性的影響

        多項研究表明,精神壓力與肌鈣蛋白的升高存在一定的相關(guān)性,在應(yīng)激情況下可造成肌鈣蛋白水平增加,牙周炎是年輕人精神壓力的危險因素,牙周炎增加了病人的精神壓力,從而進(jìn)一步導(dǎo)致血清肌鈣水平升高45-46。

        5 小結(jié)

        綜上所述,牙周炎病人牙周致病菌及其相關(guān)因子可升高血清肌鈣水平,升高的肌鈣蛋白促進(jìn)冠狀動脈鈣化,從而導(dǎo)致心肌損傷。因此,水平升高的肌鈣蛋白可能使牙周炎病人患動脈粥樣硬化及其他心血管疾病的風(fēng)險增加。今后還需更多的動物模型及臨床試驗深入探討牙周炎與肌鈣蛋白的關(guān)系,進(jìn)而為牙周炎與系統(tǒng)性疾病的關(guān)系提供依據(jù)。
