[摘要] 目的 分析二甲雙胍通過降低miR-34a抵抗心肌缺血再灌注損傷的應用效果。方法 選擇周齡6~8周的清潔級雄性SD大鼠60只作為研究對象,根據區(qū)組隨機化法將其分成3個組別(參照組、手術組、二甲雙胍組),其中參照組不對冠狀動脈左前降支進行結扎,手術組在對冠狀動脈左前降支結扎45 min后,再進行灌注,二甲雙胍組在進行與手術組相同操作的同時,給予其二甲雙胍,觀察并對比3組SD大鼠的血清生化指標與miR-34a水平。 結果 二甲雙胍組肌紅蛋白、α羥丁酸脫氫酶、乳酸脫氫酶、肌酸激酶、肌酸激酶MB以及miR-34a數值分別為(99.02±24.78)ng/mL、(320.41±29.73)U/L、(327.83±30.74)U/L、(1 671.92±570.24)U/L、(626.31±227.72)U/L以及(5.16±0.44),明顯優(yōu)于參照組,差異有統(tǒng)計學意義(t=19.505,P=0.000;t=6.069,P=0.000;t=11.883,P=0.000;t=12.136,P=0.000;t=8.994,P=0.000;t=14.769,P=0.000)。結論 二甲雙胍可有效調節(jié)miR-34a的表達,對于改善血清生化指標具有積極意義,可以控制心肌缺血再灌注損傷。

[Abstract] Objective To analyze the effect of metformin on miR-34a resistance to myocardial ischemia-reperfusion injury. Methods Sixty clean male SD rats aged 6 to 8 weeks were selected as the research objects, and they were divided into three groups according to the block randomization method(reference group, surgery group, metformin group), in the reference group, the left anterior descending coronary artery was not ligated. In the surgery group, the left anterior descending coronary artery was ligated 45 minutes before reperfusion. The metformin group was given the same operation as the surgery group, and was given metformin. Serum biochemical indicators and miR-34a levels in SD rats were observed. Results The values of myoglobin, α-hydroxybutyrate dehydrogenase, lactate dehydrogenase, creatine kinase, creatine kinase MB and miR-34a in the metformin group were (99.02±24.78) ng/mL, (320.41±29.73) U/L, (327.83±30.74) U/L, (1 671.92±570.24) U/L, (626.31±227.72) U/L, and (5.16±0.44) were significantly better than the reference group with significant differences (t=19.505, P=0.000; t=6.069, P=0.000; t=11.883, P=0.000; t=12.136, P=0.000; t=8.994, P=0.000; t=14.769, P=0.000). Conclusion Metformin can effectively regulate the expression of miR-34a, has positive significance for improving serum biochemical parameters, can control myocardial ischemia-reperfusion injury, and is worthy of further research and promotion.

心血管病患者的致死原因之一為急性心肌梗死,其病癥救治的關鍵在于梗死動脈的疏通與心肌血液供應的恢復,但是根據相關研究顯示[1],心肌缺血情況得以恢復血流灌注時,會加重患者心肌損傷的嚴重性,極易引發(fā)患者發(fā)生心肌頓抑、心律失常以及心功能不全等并癥狀,即心肌缺血再灌注損傷。因此,在恢復患者冠狀動脈血流通暢的同時,怎樣避免心肌缺血再灌注損傷情況的發(fā)生,成為心血管疾病臨床研究的熱點[2]。近年來,隨著醫(yī)療技術的不斷發(fā)展,相關文獻報道[3],miRNAs作為堿基組成的一種內源性非編碼小RNA,能夠對RNA降解起到促進效果,并可控制mRNA翻譯,對于靶基因的表達具有調控作用。因此,該研究針對二甲雙胍通過降低miR-34a抵抗心肌缺血再灌注損傷的應用效果進行分析,現報道如下。
