[摘要] 目的 研究小兒消化道異物經(jīng)胃鏡下取出術(shù)的臨床配合要點及效果。方法 方便選取該院2016年8月—2018年12月收治的124例實施經(jīng)胃鏡下取出術(shù)的小兒消化道異物患兒作為研究對象，依照奇偶數(shù)法進行分組，各62例，對照組予以常規(guī)護理，研究組予以優(yōu)質(zhì)護理，對兩組異物清除效果及心理狀況變化、并發(fā)癥發(fā)生情況進行比較。 結(jié)果 對照組患兒的異物清除率93.5%比研究組100%低，組間差異有統(tǒng)計學意義（χ2=4.133 ，P=0.042 ）;兩組術(shù)前SAS評分相比，差異無統(tǒng)計學意義（t=0.641 ，P=0.523）;術(shù)中與術(shù)后的SAS評分經(jīng)比較，研究組均比對照組低，組間顯示出的差異有統(tǒng)計學意義（t=13.464、20.934，P<0.05）;對照組并發(fā)癥發(fā)生率與研究組的并發(fā)癥發(fā)生率展開對比，研究組比對照組低，差異有統(tǒng)計學意義（χ2=5.522，P=0.019）。 結(jié)論 小兒消化道異物經(jīng)胃鏡下取出術(shù)中實施優(yōu)質(zhì)護理有利于改善患兒負性情緒，提高手術(shù)成功率。

Clinical Cooperation of Gastroscopic Removal of Foreign Bodies from Digestive Tract in Children

[Abstract] Objective To study the key points of clinical cooperation and effect of gastroscopic removal of foreign bodies from digestive tract in children. Methods Convenient election 124 children with pediatric digestive tract foreign bodies admitted to the hospital from August 2016 to December 2018 were enrolled in the study. According to the odd-even method， 62 cases were used， and the control group received routine care. The research team gave high-quality care， and compared the effects of foreign body clearance， psychological changes and complications. Results The foreign body clearance rate of the control group was 93.5% lower than that of the study group 100.00%. The difference between the groups was statistically significant（χ2=4.133， P=0.042 ）. There was no statistically significant difference between the two groups before the preoperative SAS score（t=0.641， P=0.523）; Compared with the postoperative SAS scores， the study group was lower than the control group， and the difference between the groups was statistically significant （t=13.464 ， 20.934 ， P<0.05）; The incidence of complications in the control group was compared with the incidence of complications in the study group. The study group was lower than the control group， and the difference was statistically significant （χ2=5.522， P=0.019）. Conclusion The implementation of high-quality care in children with digestive tract foreign body after endoscopic removal is beneficial to improve negative emotions and improve the success rate of surgery.

該次實驗中，研究組患兒異物清除率100.0%高于對照組患兒93.5%，其術(shù)中、術(shù)后SAS評分（38.02±4.87）分、（36.03±2.53）分、并發(fā)癥發(fā)生率6.5%則低于對照組患兒（50.24±5.23）分、（49.18±4.25）分、19.4%（P<0.05）。龐勤輝等[14]在《經(jīng)內(nèi)鏡鉗取小兒上消化道異物的配合技巧及護理》一文中選擇經(jīng)電子胃鏡診斷和治療的278例小兒消化異物，經(jīng)內(nèi)鏡鉗取小兒上消化道異物取出術(shù)治療的同時配合臨床護理，結(jié)果顯示275例成功完成胃鏡下鉗取術(shù)，僅2例因十二指腸降部塑料長膀嵌頓時間過長而治療失敗，異物清除率為98.9%，患兒術(shù)中、術(shù)后SAS評分分別為（39.05±5.16）分、（35.18±2.49）分、并發(fā)癥發(fā)生率為7.3%。提示開展小兒消化道異物取石術(shù)過程中，做好術(shù)中配合和臨床護理干預，有利于提高手術(shù)成功率，降低并發(fā)癥發(fā)生率，改善患兒心理狀況。

綜上所述，小兒消化道異物經(jīng)胃鏡下取出術(shù)中護理人員良好的手術(shù)配合有利于改善患兒負性情緒，提高手術(shù)成功率。但因為該次實驗選擇的病例樣本較少，未對患兒預后情況進行分析和評價，后期還需要相關(guān)研究學者通過增加樣本數(shù)量，延長隨訪時間的方式進行補充和完善。
