[摘要]在醫(yī)學(xué)技術(shù)突飛猛進(jìn)的背景下,醫(yī)學(xué)繼續(xù)教育成為醫(yī)學(xué)教育的重要組成部分。耳鼻咽喉頭頸外科以其專(zhuān)科性強(qiáng)、輔助器械和設(shè)備使用多、多學(xué)科交叉等特點(diǎn)成為醫(yī)學(xué)繼續(xù)教育的重點(diǎn)和難點(diǎn)。目前的醫(yī)學(xué)繼續(xù)教育形式從大體上可以分為傳統(tǒng)形式和網(wǎng)絡(luò)相關(guān)的新的醫(yī)學(xué)繼續(xù)教育形式,前者包括專(zhuān)題會(huì)議、疑難病例討論會(huì)、專(zhuān)題學(xué)習(xí)班、學(xué)術(shù)沙龍和雜志專(zhuān)欄等,后者有公眾微信平臺(tái)和慕課等。醫(yī)學(xué)繼續(xù)教育在耳鼻咽喉頭頸外科專(zhuān)科培訓(xùn)上存在機(jī)遇和挑戰(zhàn),完善培訓(xùn)內(nèi)容、建立系統(tǒng)性的培訓(xùn)課程和標(biāo)準(zhǔn)化的培訓(xùn)評(píng)價(jià)體系是未來(lái)發(fā)展的方向。

Exploration and practice of continuing medical education in specialist training in otolaryngology head and neck surgery

[Abstract]With the rapid development of medical technology,continuing medical education has become an important part in the medical education system.Otolaryngology head and neck surgery is characterized by its highspecialization,strong demand on facilities and devices,and interdisciplinarity,which makes it one of the most difficult majors in the continuing medical education.Currently,the forms of continuing medical education can be generally divided into two categories:traditional form,including seminar,trouble case consultation,symposium,academic salon and special column in medical journals,and internet-related new form.The latter includes pubic WeChat platform,massive open online course (MOOC) and so on.The opportunity and challenge of continuing medical education in specialist training in otolaryngology head and neck surgery will promote its improvement in systematic arrangement of the course and standardization of its evaluation system.

隨著醫(yī)學(xué)科學(xué)技術(shù)日新月異,臨床疾病的診療理念和技術(shù)不斷更新,醫(yī)學(xué)教育從學(xué)校教育延伸到畢業(yè)后的繼續(xù)教育。醫(yī)學(xué)繼續(xù)教育越來(lái)越成為醫(yī)學(xué)教育不可或缺的組成部分,它的良好實(shí)踐是造就我國(guó)高素質(zhì)、高水平醫(yī)療隊(duì)伍的重要環(huán)節(jié)。經(jīng)過(guò)近十年的發(fā)展,住院醫(yī)師規(guī)范化培訓(xùn)已經(jīng)形成全國(guó)醫(yī)學(xué)生畢業(yè)后培訓(xùn)體系,目標(biāo)是培訓(xùn)具有良好職業(yè)道德、扎實(shí)醫(yī)學(xué)理論知識(shí)和臨床技能,能獨(dú)立、規(guī)范地承擔(dān)本專(zhuān)業(yè)常見(jiàn)病、多發(fā)病診療工作的臨床醫(yī)師[1]。但臨床分科日趨精細(xì),住院醫(yī)師規(guī)范化培訓(xùn)結(jié)束后出現(xiàn)了專(zhuān)科培訓(xùn)的欠缺。目前各專(zhuān)業(yè)的醫(yī)學(xué)繼續(xù)教育工作由各個(gè)醫(yī)院、科室,中華醫(yī)學(xué)會(huì)及地方醫(yī)學(xué)會(huì)下屬各分會(huì)以及中國(guó)醫(yī)師協(xié)會(huì)各分會(huì)等以專(zhuān)題會(huì)議、學(xué)術(shù)沙龍、網(wǎng)絡(luò)課程等形式開(kāi)展,但這還遠(yuǎn)遠(yuǎn)不能滿足日益增長(zhǎng)的專(zhuān)業(yè)領(lǐng)域進(jìn)展和臨床的需求。加之,我國(guó)醫(yī)學(xué)教育處于專(zhuān)科、本科和研究生(碩士和博士)多系統(tǒng)并存的狀況,各級(jí)醫(yī)院醫(yī)生水平參差不齊。加強(qiáng)醫(yī)學(xué)繼續(xù)教育在各醫(yī)學(xué)專(zhuān)科培訓(xùn)中的地位和作用是目前亟需解決的。
耳鼻咽喉-頭頸外科是在原耳鼻喉科基礎(chǔ)上發(fā)展起來(lái)的醫(yī)學(xué)三級(jí)學(xué)科,目前包括耳科、鼻科、咽喉科和頭頸外科等四個(gè)亞科。因?yàn)榻M織結(jié)構(gòu)深在和精細(xì),內(nèi)鏡技術(shù)和顯微手術(shù)應(yīng)用是該學(xué)科的特點(diǎn)。此外,還與多個(gè)其他三級(jí)學(xué)科有交叉,其中涵蓋的前顱底和側(cè)顱底外科和神經(jīng)外科有交叉,氣管和食管相關(guān)疾病及處理與胸外科等有交叉,頭頸外科涉及到血管吻合,與血管外科有分工合作等等。目前的耳鼻咽喉-頭頸外科已遠(yuǎn)遠(yuǎn)超出“四炎一聾(中耳炎、鼻炎、咽炎、喉炎和耳聾)”的范疇,專(zhuān)業(yè)內(nèi)容涉及面廣,對(duì)專(zhuān)業(yè)知識(shí)技能要求高[2]。本文旨在以耳鼻咽喉-頭頸外科為例,拋磚引玉,對(duì)醫(yī)學(xué)繼續(xù)教育在專(zhuān)科培訓(xùn)中的探索與實(shí)踐作一探討。
1耳鼻咽喉-頭頸外科各亞科的專(zhuān)科醫(yī)師培訓(xùn)特點(diǎn)及要求
美國(guó)醫(yī)學(xué)會(huì)把繼續(xù)醫(yī)學(xué)教育定義為由教育活動(dòng)組成,旨在維持、發(fā)展及增進(jìn)知識(shí)、技能、醫(yī)療人員的實(shí)際工作及一名醫(yī)師用以提供對(duì)患者、公眾及本專(zhuān)業(yè)服務(wù)的各種職業(yè)關(guān)系[3]。專(zhuān)科醫(yī)師培訓(xùn)制度是指管理專(zhuān)科醫(yī)師的辦事規(guī)程與準(zhǔn)則,包括專(zhuān)科醫(yī)師培訓(xùn)制度、準(zhǔn)入制度和管理制度。它是國(guó)際醫(yī)學(xué)界公認(rèn)的醫(yī)學(xué)教育制度,中國(guó)從20世紀(jì)90年代初全面推行住院醫(yī)師規(guī)范化培訓(xùn)制度,但至今尚未建立統(tǒng)一、規(guī)范的專(zhuān)科醫(yī)師培訓(xùn)制度[4]。耳鼻咽喉頭頸外科又有細(xì)分,各亞科的專(zhuān)科培訓(xùn)繼續(xù)教育特點(diǎn)總結(jié)如下。
