中間型單核細胞及TREM-1在點滴狀銀屑病中的表達

目的: 檢測點滴狀銀屑病患者中間型單核細胞比例及其細胞表面觸發(fā)受體(TREM-1)的表達,探討其在點滴狀銀屑病疾病過程中的作用。方法: 采集28例點滴狀銀屑病患者和24例健康對照者外周血,流式細胞術(shù)檢測中間型單核細胞及TREM-1+中間型單核細胞比。ELISA檢測血清游離TREM-1(sTREM-1)濃度。計算中間型單核細胞與PASI評分及抗鏈球菌溶血素O(ASO)的相關(guān)性。結(jié)果: 點滴狀銀屑病患者中間型單核細胞比例及sTREM-1水平分別為13.5±4.8%和277.8 pg/mL,顯著高于健康對照組的3.4±1.1%和162.6 pg/mL,(P<0.05);患者組TREM-1平均熒光強度及陽性中間型核細胞頻率分別為24.8±7.2和80.4±6.8%,低于健康對照組的35.3±5.1和95.6±5.7%,(P<0.05)。患者中間型單核細胞比例與PASI,ASO水平正相關(guān)。結(jié)論: 中間型單核細胞及其表面TREM-1受體可能參與點滴狀銀屑病的疾病過程。

點滴狀銀屑病除了自身反應(yīng)性T細胞活化和表皮角質(zhì)形成細胞異常增殖,固有免疫系統(tǒng)因細菌、真菌等微生物感染引起過度活化在疾病過程中也發(fā)揮重要作用。1在炎癥反應(yīng)過程中,中間型單核細胞具有較強的抗原提呈和加工能力,具有較高的活化炎癥反應(yīng)的潛能;2單核細胞表面髓系細胞觸發(fā)受體-1(TREM-1)具有激發(fā)和放大炎癥反應(yīng)的作用。3本次研究通過檢測點滴狀銀屑病患者外周血中間型單核細胞比例及其TREM-1表達,并分析其與點滴狀銀屑病其他診斷指標或致病物質(zhì)(如:抗鏈球菌溶血素O)的相關(guān)性,闡明中間型單核細胞在點滴狀銀屑病疾病過程中的作用。
1.1研究對象 選取點滴狀銀屑病患者28例,其中男12例、女16例,年齡(23.5±12.1)歲,同時設(shè)健康對照24例,其中男11例、女13例,年齡(24.5±14.2)歲,無自身免疫性疾病或近期感染史。兩組間性別、年齡比較差異無統(tǒng)計學意義(P>0.05)。本次研究經(jīng)醫(yī)院倫理委員會批準,征得患者知情同意,采集外周血3 mL。
1.2主要儀器與試劑 異硫氰酸熒光素(FITC)鼠抗人-CD14、葉綠素蛋白偶聯(lián)物(PerCP-cy5.5)鼠抗人-CD16單克隆抗體及同型對照購自BD公司,PE-鼠抗人TREM-1及同型對照購自R&D公司;流式細胞儀FACSCanto為 BD公司產(chǎn)品??咕?LL-37及sTREM-1酶聯(lián)免疫吸附試驗(ELISA)檢測試劑盒分別購自Hycult Biotech公司和R&D公司。
1.3方法
1.3.1流式細胞術(shù)檢測細胞表面受體 每100μL外周血加入10μL PerCP-cy5.5-抗人CD16、FITC-抗人CD14和PE-抗人TREM-1及同型對照,室溫避光孵育30 min,經(jīng)紅細胞裂解液破壞紅細胞后,用1mL磷酸鹽緩沖液(PBS)洗滌兩次,1500 rpm離心6 min,棄上清,加入400μL PBS重懸細胞。FACSCanto采用前向散射光(FS)和側(cè)向散射光(SS)區(qū)別單核細胞(圖1aR1),以中間型單核細胞設(shè)門(圖1b R4),測定中間型單核細胞/單核細胞百分率,以及TREM-1+中間型單核細胞/中間型單核細胞百分率。
1.3.2銀屑病相關(guān)診斷或評估指標 計算所有患者銀屑病皮損面積和嚴重程度指數(shù)(PASI)。外周血經(jīng)500×g離心5 min,分離血漿,儲存在-80℃低溫冰箱。按ELISA檢測法檢測sTREM-1,每標本測2次,取均值??规溓蚓苎豋(ASO)采用免疫比濁法,BNProSPec特定蛋白分析儀及配套試劑檢測。
1.4統(tǒng)計學方法 用Stata 7.0統(tǒng)計軟件對數(shù)據(jù)進行正態(tài)性檢驗,本次研究全部為計量資料,正態(tài)性分布用ˉx±s表示,兩組比較采用students't-test檢驗;偏態(tài)資料用中位數(shù)(M)表示,兩組比較采用Mann-Whitney U test,作雙側(cè)檢驗。線性相關(guān)系數(shù)計算采用Pearson相關(guān)系數(shù)。取P<0.05作為差異有統(tǒng)計學意義的標準。
2.1流式細胞術(shù)檢測中間型單核細胞比例 點滴狀銀屑病患者中間型單核細胞比例(3.5±4.8%)高于健康對照組(3.4±1.1%),差異有統(tǒng)計學意義(P<0.01)(圖1d)。

2.2中間型單核細胞TREM-1表達 流式細胞儀以中間型單核細胞設(shè)門,測定TREM-1+中間型單核細胞/中間型單核細胞百分率(圖2a)和TREM-1 MFI平均熒光強度(圖2b)。

點滴狀銀屑病患者中間型單核細胞TREM-1陽性比例及平均熒光強度(MFI)均低于健康對照組,差異有統(tǒng)計學意義(P<0.05),結(jié)果見表1。

2.3ELISA檢測血清sTREM-1濃度 點滴狀銀屑病患者血清sTREM-1濃度為152.5~372.4 pg/mL,平均為277.8 pg/mL,對照組濃度為124.3~209.7 pg/ m L,平均為162.6 pg/m L。經(jīng)Mann-Whitney U test檢驗,差異有統(tǒng)計學意義(P<0.05)。
2.4患者中間型單核細胞比例與臨床數(shù)據(jù)相關(guān)性分析 點滴狀銀屑病患者中間型單核細胞比例與血清sTREM-1濃度、ASO、PASI呈正相關(guān)(圖3)。

單核細胞根據(jù)其表面標記CD14、CD16分為3個亞群:經(jīng)典型(CD14highCD16-)、中間型(CD14highCD16+)和非經(jīng)典型(CD14lowCD16++),分別約占外周血單核細胞的85%、5%和10%,不同的亞群在表型、功能及炎癥活化潛能方面存在明顯差異。5經(jīng)典型具有很強的吞噬功能;非經(jīng)典型高表達細胞骨架能動性相關(guān)基因,能識別炎癥信號并迅速遷移到炎癥部位,浸潤組織后分化為巨噬細胞。中間型單核細胞高表達編碼MHC-II類分子相關(guān)基因(如HLA-DR和CD74),具有較強的抗原提呈和加工能力,并在T細胞活化、增殖及抗原提呈過程中起重要作用。此外,還表達促血管生成的相關(guān)分子,具有促血管生成的特性。4本次研究發(fā)現(xiàn),點滴狀銀屑病患者外周血中間型單核細胞比例明顯升高。Fujisawa等6研究證實,泛發(fā)性膿皰型銀屑病患者外周血中間型單核細胞比例明顯升高。
TREM-1表達于中性粒細胞和單核巨噬細胞等細胞表面,具有激發(fā)和放大炎癥反應(yīng)的作用,在炎癥反應(yīng)過程中,通過基質(zhì)金屬蛋白酶的水解,膜表面TREM-1從細胞表面脫落下來,形成sTREM-l,從而下調(diào)炎癥反應(yīng),是機體在炎癥反應(yīng)中的一種自身保護調(diào)節(jié)。7血漿sTREM-1濃度可用于臨床膿毒癥的早期診斷以及評估病情嚴重程度。8在急性胰腺炎、炎性腸病、類風濕關(guān)節(jié)炎中,血清sTREM-1濃度均不同程度升高。9中性粒細胞和單核細胞表面TREM-1表達在炎癥反應(yīng)中升高或降低與感染的類型、疾病的炎癥嚴重程度及病程有關(guān)。10,11本次調(diào)查發(fā)現(xiàn),點滴狀銀屑病患者血清sTREM-1升高,中間型單核細胞TREM-1表達明顯下降,說明中間型單核細胞TREM-1參與炎癥反應(yīng)。
ASO是診斷鏈球菌感染的敏感指標。較多研究表明,12點滴狀銀屑病與細菌感染有關(guān),最常見的細菌包括葡萄球菌和鏈球菌等,機體其他部位的感染,如扁桃體、腸道、牙周炎也會誘發(fā)銀屑病。中間型單核細胞在感染性疾病和多種自身免疫性疾病過程中發(fā)揮重要作用,包括敗血癥、類風濕關(guān)節(jié)炎(RA)、多發(fā)性硬化癥、1型糖尿病等。13,14本次研究證實,點滴狀銀屑病患者外周血中間型單核細胞比例與ASO濃度正相關(guān)。
本次研究還發(fā)現(xiàn),點滴狀銀屑病患者外周血中間型單核細胞比例與PASI評分正相關(guān),F(xiàn)ujisawa等6用吸附法去除中間型單核細胞可明顯改善患者臨床癥狀。在銀屑病鼠模型中用白細胞毒素(LtxA)促進中間型單核細胞凋亡具有顯著的治療作用。15因此,通過調(diào)控單核細胞凋亡或向非炎性單核細胞極化可能是治療點滴狀銀屑病的新策略。有研究表明,TREM-1活化可調(diào)節(jié)巨噬細胞向促炎型(M1)極化,16用TREM -1活化性抗體活化單核細胞TREM-1,可誘導單核細胞分化為不成熟DC(iDC),導致T細胞增殖。17但TREM-1在單核細胞向中間型極化過程中的作用有待于進一步研究。
綜上所述,點滴狀銀屑病患者外周血中間型單核細胞比例升高,并與PASI具有相關(guān)性,其在銀屑病病情和療效評估中的作用需進一步采用大樣本驗證。單核細胞TREM-1參與點滴狀銀屑病發(fā)病過程,導致TREM-1活化的機制也需進一步研究。調(diào)控外周血單核細胞極化及其TREM-1表達可能是治療點滴狀銀屑病的新途徑。

Expression of TREM-1 and intermediatemonocytes in patientsw ith psoriasis guttata

Objective:To detect the levelof TREM-1 and intermediatemonocytes in the patientswith psoriasis guttata,in order to determine TREM-1's role in the pathogenesis process of psoriasis guttata.M ethods:The peripheral blood was collected from 28 patients and 24 healthy controls.The percentage of intermediate monocytes and TREM-1 expression were analyzed through flow cytometry.The level of sTREM-1 in serum was detected by ELISA.The correlation between intermediatemonocytes,the levels of PASIand ASOwere assessed.Results:The percentage of intermediatemonocytes and the level of sTREM-1 in patientswere 13.5± 4.8%and 277.8 pg/mL,which was higher than those of healthy controls(3.4±1.1%,162.6 pg/mL),with significantly differences(P<0.05).The positivity rate of TREM-1 in intermediatemonocytes(80.4±6.8%)and the fluorescence intensity of TREM-1(24.8±7.2)were lower than those of healthy controls(95.6±5.7% and 35.3±5.1)(P<0.05).There was positive correlation between the percentage of intermediate monocytes and the levels of PASIand ASO(P<0.05).Conclusion:TREM-1 and intermediate monocytesmay be involved in the progress of the disease.
