摘要:目的? 分析自主呼吸狀態(tài)下光電容積脈搏波法對(duì)健康人群心率變異性。方法? 選取2016年10月~2018年9月我校本科生健康志愿者64名,所有志愿者均行心電圖(ECG)及光電容積脈搏波法(PPG)檢測(cè),比較3 min的ECG與PPG檢測(cè)HRV時(shí)的RR間期標(biāo)準(zhǔn)差(SDNN)、RR間期差值均方根值(RMSSD)、低頻功率與標(biāo)準(zhǔn)化高頻功率的比值(LF/HF),并分析ECG與PPG檢測(cè)的心率變異性參數(shù)的相關(guān)性與一致性。結(jié)果? 64例志愿者3 min的ECG與PPG心率變異性參數(shù)比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。經(jīng)Pearson相關(guān)性分析發(fā)現(xiàn),PPG檢測(cè)SDNN、RMSSD、LF/HF與ECG各指標(biāo)呈正相關(guān)(SDNN:r=0.999,P<0.001;RMSSD:r=0.998,P<0.001;LF/HF:r=0.998,P<0.001);經(jīng)可靠性分析發(fā)現(xiàn),PPG用于檢測(cè)健康人群心率變異性與ECG一致性較高(ICC=0.915,95% CI:0.878~0.943,P<0.001)。結(jié)論? 健康人群自主呼吸狀態(tài)下光電容積脈搏波法可用于心率變異性的檢測(cè),臨床上有較好的應(yīng)用價(jià)值。

Abstract:Objective? To analyze the heart rate variability of healthy people under the photoresonance pulse wave method under spontaneous breathing.Methods? From October 2016 to September 2018, 64 undergraduate health volunteers were selected.All volunteers underwent electrocardiogram (ECG) and photoplethysmography (PPG), and compared with 3 min ECG and PPG for HRV. RR interval standard deviation (SDNN), RR interval rms value (RMSSD), ratio of low frequency power to normalized high frequency power (LF/HF), and analysis of heart rate variability parameters detected by ECG and PPG Relevance and consistency.Results? There were no significant differences in ECG and PPG heart rate variability parameters between the 64 volunteers for 3 min (P>0.05). According to Pearson correlation analysis, PPG detection of SDNN, RMSSD, LF/HF and ECG indicators were positively correlated (SDNN:r=0.999,P<0.001; RMSSD:r=0.998,P<0.001; LF/HF: r=0.998,P<0.001);according to the reliability analysis, PPG was used to detect heart rate variability and ECG consistency in healthy people (ICC=0.915,95%CI:0.878-0.943,P<0.001).Conclusion? Photoelectric volume pulse wave method can be used for the detection of heart rate variability in healthy people under spontaneous breathing. It has good clinical application value.

心率變異性(heart rate variability,HRV)是指通過對(duì)心臟搏動(dòng)時(shí)間間隔的微小變化進(jìn)行連續(xù)分析,以反映自主神經(jīng)系統(tǒng)中交感神經(jīng)與副交感神經(jīng)活動(dòng)的一項(xiàng)指標(biāo)[1]。心電圖(electrocardiogram,ECG)主要描記的是心臟收縮及舒張過程中體表所表現(xiàn)的電勢(shì)差,根據(jù)心電圖心臟節(jié)律探測(cè)正常竇性節(jié)律與異位節(jié)律[2]。但心電圖在測(cè)量過程中,需專業(yè)的醫(yī)護(hù)人員操作,使其應(yīng)用受到限制。而光電容積脈搏波(PPG)信號(hào)的獲取主要通過光照射皮膚或組織表面,并透過組織,由血液吸收,形成光能量及散射光能量,且隨著血流搏動(dòng)血容量的變化而發(fā)生變化,最終利用光電探測(cè)器探測(cè)獲取透射光或散射光[3]。光電容積脈搏波呈現(xiàn)脈動(dòng)性變化,且與心臟搏動(dòng)密切相關(guān),可有效反映心輸出量、血氧飽和度、動(dòng)脈順應(yīng)性、外周循環(huán)阻力等與心血管系統(tǒng)相關(guān)的多種狀態(tài)信息;同時(shí),PPG波形測(cè)量無需復(fù)雜、特殊的儀器設(shè)備,具有無創(chuàng)、操作簡(jiǎn)單、適應(yīng)性強(qiáng)、性能穩(wěn)定等優(yōu)勢(shì),PPG監(jiān)測(cè)只需將指套套于手指末梢,不僅為用戶提供較高的舒適性,還方便非專業(yè)人員的操作,廣泛應(yīng)用于監(jiān)護(hù)及移動(dòng)生理參數(shù)監(jiān)測(cè)等領(lǐng)域中[4]。本文主要分析自主呼吸狀態(tài)下光電容積脈搏波法對(duì)健康人群心率變異性,現(xiàn)報(bào)道如下。
