摘 要 目的：探索基于格林（Greens）模式的醫(yī)院-社區(qū)-家庭三維健康教育模式在老年慢性病患者安全用藥中的應(yīng)用。方法：選取2019年1月至2021年1月在上海黃浦區(qū)豫園街道社區(qū)衛(wèi)生服務(wù)中心接受健康教育的老年慢性病患者92例為研究對(duì)象。按照隨機(jī)數(shù)字表法分為格林組與對(duì)照組，每組46例。對(duì)照組有女性20例，男性26例；年齡為60～81歲，平均年齡為（70.11±8.73）歲；病程為4～16年，平均病程為（8.35±1.63）年。格林組有女性19例，男性27例；年齡為62～83歲，平均年齡為（71.02±8.13）歲；病程為4～15年，平均病程為（8.48±1.43）年。對(duì)照組選擇常規(guī)健康教育實(shí)施管理，格林組選擇基于PRECEDE-PROCEED模式的醫(yī)院-社區(qū)-家庭三維健康教育實(shí)施管理。比較兩組安全用藥知識(shí)、慢性病防治信念評(píng)分及用藥依從性。結(jié)果：格林組安全用藥知識(shí)、慢性病防治信念評(píng)分明顯高于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）；格林組用藥依從率明顯高于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論：基于格林模式的醫(yī)院-社區(qū)-家庭三維健康教育模式可有效提高老年慢病人群安全用藥知識(shí)，同時(shí)能提升患者用藥依從率。

Application of three-dimensional health education mode based on Greens mode in the safe medication of elderly patients with chronic diseases

ABSTRACT Objective： To explore the application of hospital-community-family three-dimensional health education mode based on Greens mode in the safe medication of elderly patients with chronic diseases. Methods： Ninety-Two elderly patients with chronic diseases who received health education at Yuyuan Community Health Service Center of Huangpu District from January 2019 to January 2021 were selected as the research objects. According to the random number table method， they were divided into a Green group and a control group with 46 cases in each group. In the control group， there were 20 females and 26 males； the age ranged from 60 to 81 years old， with an average age of （70.11±8.73） years old； the course of disease was 4～16 years， and the average course of disease was （8.35±1.63） years.There were 19 females and 27 males in the Green group； the age ranged from 62 to 83 years old， with an average age of （71.02±8.13） years old； the course of disease was 4～15 years， and the average course of disease was （8.48±1.43） years. The control group chose regular health education and management， and the Green group chose the hospital-community-family three-dimensional health education management based on the PRECEDEPROCEED mode. The knowledge of safe medication， belief score of chronic disease prevention and treatment and medication compliance were compared between the two groups. Results： The scores of the knowledge safe medication and chronic disease prevention and treatment belief of the Green group were significantly higher than those of the control group， and the difference were statistically significant（P<0.05）； the medication compliance rate of the Green group was significantly higher than that of the control group， and the difference was statistically significant（P<0.05）. Conclusion： The hospital-community-family threedimensional health education mode based on Greens mode can effectively improve the knowledge of safe medication of elderly people with chronic diseases， and at the same time can improve the medication compliance rate of patients.
