        隨著小切口冠狀動(dòng)脈旁路移植術(shù)(CABG)日益成熟,將小切口CABG和經(jīng)皮冠狀動(dòng)脈介入治療(PCI)相結(jié)合的雜交冠狀動(dòng)脈血運(yùn)重建術(shù)(HCR)越來(lái)越引起了人們的關(guān)注[1-9]。HCR可以分為“一站式”和“分站式”雜交技術(shù),“一站式”HCR需要在專(zhuān)門(mén)的雜交手術(shù)室內(nèi)進(jìn)行,一次性麻醉下完成小切口CABG和PCI;“分站式”HCR是將小切口CABG和PCI分期完成。我們自2012年開(kāi)始進(jìn)行了73例“分站式”HCR治療冠狀動(dòng)脈多支血管病變的嘗試,現(xiàn)將其圍手術(shù)期和中期臨床隨訪結(jié)果進(jìn)行報(bào)道,探討該手術(shù)的安全性及臨床應(yīng)用前景和價(jià)值。

        1 資料與方法

        1.1 病例選擇

        選擇2012-05至2014-08在我院心臟中心因冠狀動(dòng)脈多支血管病變接受擇期“分站式”HCR的73例患者,其中男性50例(68.5%),女性23例(31.5%),平均年齡(61.1±10.7)歲,術(shù)前情況詳見(jiàn)表1。入選標(biāo)準(zhǔn):冠狀動(dòng)脈造影顯示為多支病變,其中左前降支為分叉病變、嚴(yán)重鈣化或完全閉塞病變等不適合行介入治療,同時(shí)非左前降支病變適宜進(jìn)行PCI。

        1.2 研究方法

        “分站式”HCR流程:先行小切口CABG,全麻雙腔氣管插管。仰臥位,左前胸第4或第5肋間切口約長(zhǎng)5 cm,進(jìn)胸后單肺通氣。放置懸吊式乳內(nèi)動(dòng)脈牽開(kāi)系統(tǒng)(FEHLING),直視下獲取左乳內(nèi)動(dòng)脈。切開(kāi)心包,確定左前降支吻合位置,肝素化(1 mg/kg)后懸吊心包,用負(fù)壓式心臟穩(wěn)定器固定前降支,切開(kāi)冠狀動(dòng)脈后放置分流栓,使用8-0 prolene 線完成左乳內(nèi)動(dòng)脈和左前降支的吻合。術(shù)后6 h起開(kāi)始給予肝素靜脈推注,每次20 mg,每6 h重復(fù)1次,直至手術(shù)次日晨同時(shí)服用阿司匹林(100 mg,1次/天)和氯吡格雷(75 mg,1次/天)雙聯(lián)抗血小板治療。

        小切口CABG后非前降支冠狀動(dòng)脈的PCI:小切口CABG術(shù)后3~7天,在介入導(dǎo)管室先行冠狀動(dòng)脈造影,觀察左乳內(nèi)動(dòng)脈-左前降支旁路血管情況,證實(shí)其通暢后對(duì)非左前降支病變行PCI治療并置入支架。PCI術(shù)后,給予阿司匹林(100 mg,1次/天)和氯吡格雷(75 mg,1次/天),需服用1年。

        隨訪和研究終點(diǎn):出院后每3個(gè)月通過(guò)電話和問(wèn)卷方式進(jìn)行隨訪,術(shù)后每年進(jìn)行1次超聲心動(dòng)圖、X線胸片和心電圖的常規(guī)檢查,如患者出現(xiàn)心肌缺血表現(xiàn),則進(jìn)行冠狀動(dòng)脈增強(qiáng)計(jì)算機(jī)斷層攝影術(shù)(CTA)或冠狀動(dòng)脈造影檢查。隨訪的終點(diǎn)是主要不良心腦血管事件(MACCE)的發(fā)生,包括:死亡、心肌梗死(典型癥狀、心電圖和血清標(biāo)志物變化)、心絞痛復(fù)發(fā)、腦血管事件(腦卒中或短暫性腦缺血發(fā)作)、以及靶血管病變或反復(fù)再血管化。

        1.3 統(tǒng)計(jì)學(xué)分析

        采用SPSS 17.0軟件進(jìn)行統(tǒng)計(jì)學(xué)處理。連續(xù)變量以均數(shù)±標(biāo)準(zhǔn)差非連續(xù)變量采用頻率(百分?jǐn)?shù)) 和率表示。

        2 結(jié)果

        2.1 圍手術(shù)期情況(表2)

        73例患者均順利接受了擇期“分站式”HCR,無(wú)圍手術(shù)期死亡。小切口CABG后距離PCI處理非左前降支靶血管病變時(shí)間(5.3±2.9)天,期間無(wú)MACCE發(fā)生,冠狀動(dòng)脈造影顯示左乳內(nèi)動(dòng)脈-左前降支旁路血管橋通暢率100%(73/73),PCI平均置入藥物洗脫支架(1.6±0.7) 枚。

        2.2 術(shù)后隨訪

        術(shù)后73例(100%)患者均獲得隨訪,隨訪時(shí)間(25.0±9.6)個(gè)月。隨訪期間MACCE發(fā)生5例(6.8%),其中死亡1例(1.4%),死于肺心病,再發(fā)心肌缺血3例(4.1%),隨訪期間需要接受CABG/PCI者1例(1.4%,因支架再狹窄,再次置入支架)。

        3 討論

        CABG和PCI是目前治療冠狀動(dòng)脈粥樣硬化性心臟病的常用方法。CABG時(shí)左乳內(nèi)動(dòng)脈至左前降支搭橋的高遠(yuǎn)期通暢率已經(jīng)得到了廣泛的認(rèn)同,但常規(guī)CABG的大創(chuàng)傷性也成為了很多患者的顧慮。而相對(duì)低創(chuàng)傷的PCI對(duì)于非左前降支病變,其近遠(yuǎn)期通暢率甚至要高于靜脈橋[3,5,6]。

        與“一站式”HCR一樣,“分站式”HCR主要適合于合并左前降支嚴(yán)重病變無(wú)法接受PCI而右冠狀動(dòng)脈主干和(或)回旋支等非左前降支病變可以進(jìn)行PCI的冠狀動(dòng)脈多支病變者,我們強(qiáng)調(diào)術(shù)前心外科醫(yī)師和心內(nèi)科介入醫(yī)師共同復(fù)習(xí)冠狀動(dòng)脈造影資料,結(jié)合患者全身情況,綜合決定是否可接受“分站式”HCR并制定具體的手術(shù)方案。

        目前,“分站式”HCR中兩種治療措施的先后順序以及間隔時(shí)間,亦無(wú)相關(guān)的指南性文獻(xiàn)[10-12]。我們的經(jīng)驗(yàn)是,對(duì)于罪犯血管為非前降支的急性冠狀動(dòng)脈綜合征患者,先行開(kāi)通犯罪血管,然后擇期行小切口CABG;除此情況之外,則先行小切口CABG再行PCI的流程更加合理。由于“分站式”HCR是將小切口CABG和PCI分期完成,兩次手術(shù)期間存在未干預(yù)血管急性閉塞的風(fēng)險(xiǎn),在實(shí)踐中我們采用了相對(duì)積極的抗凝方法和抗血小板方案:手術(shù)前阿司匹林抗血小板治療的同時(shí)加用低分子肝素抗凝,術(shù)后6 h即給予靜脈肝素抗凝,次日晨同時(shí)服用阿司匹林和氯吡格雷雙聯(lián)抗血小板治療。

        本研究結(jié)果表明,所有73例患者均采用先行小切口CABG再行PCI的手術(shù)方案,小切口CABG后距離PCI處理非左前降支靶血管病變時(shí)間(5.3±2.9)天,在等待PCI期間沒(méi)有MACCE發(fā)生,冠狀動(dòng)脈造影顯示左乳內(nèi)動(dòng)脈-左前降支旁路血管橋通暢率100%,PCI平均置入藥物洗脫支架(1.6±0.7)枚,平均處理冠狀動(dòng)脈血管(2.6±0.5)支。我們的結(jié)果表明,盡管采取相對(duì)積極的抗血小板方案,“分站式”HCR后未增加出血的風(fēng)險(xiǎn),反而由于小切口CABG的微創(chuàng)性,從而顯著減少術(shù)后用血量,在血源日益緊張的今天,這種技術(shù)無(wú)疑具有很好的臨床價(jià)值和社會(huì)價(jià)值;我們采取的將小切口CABG和PCI分期完成,即所謂“分站式”HCR策略并不顯著增加圍手術(shù)期心肌梗死和術(shù)后腎功能不全的風(fēng)險(xiǎn),小切口CABG后等待PCI期間沒(méi)有MACCE發(fā)生,說(shuō)明只要能做到規(guī)范化管理和合理使用抗血小板藥物,對(duì)于沒(méi)有雜交室的醫(yī)院,先小切口CABG后PCI分次解決左前降支病變和非左前降支病變的“分站式”HCR,對(duì)于合并左前降支病變的多支病變的冠心病患者是安全可行的,“分站式”HCR是一種可行的、安全的治療方式[13]。

        研究果表明,“一站式”HCR對(duì)多支血管病變血運(yùn)重建后療效良好,術(shù)后MACCE發(fā)生率明顯低于PCI,與CABG結(jié)果相似[14-16]。由于客觀條件的限制,在專(zhuān)門(mén)的雜交手術(shù)室尚不能普及的我國(guó)和其他發(fā)展中國(guó)家,采用“分站式”HCR更符合我國(guó)目前的國(guó)情,不增加額外的經(jīng)濟(jì)負(fù)擔(dān)和資金投入,就可能使患者享受到同樣的醫(yī)療服務(wù),“分站式”HCR有其發(fā)展空間,適用范圍廣。但是目前關(guān)于“分站式”HCR后中遠(yuǎn)期隨訪研究很少,其中遠(yuǎn)期生存率和MACCE的發(fā)生率等方面尚無(wú)定論,仍處于研究和探索中。已有的早中期隨訪研究表明,“分站式”HCR一年隨訪中MACCE的發(fā)生率低于CABG或相近,而“一站式”和“分站式”HCR之間并無(wú)區(qū)別[17]。

        我們的中期隨訪結(jié)果表明,在接受了“分站式”HCR的73例患者中,隨訪期間MACCE發(fā)生5例(6.8%),其中死亡1例(1.4%)、再發(fā)心肌缺血者3例(4.1%)、隨訪期間需要接受CABG/PCI者1例(1.4%)、腦卒中發(fā)生0例(0%)。從已有的隨訪結(jié)果分析,我們認(rèn)為“分站式”HCR具有創(chuàng)傷小、手術(shù)時(shí)間短,術(shù)后恢復(fù)快,不增加患者的住院時(shí)間、不需要建立雜交手術(shù)室等優(yōu)點(diǎn),其中期隨訪結(jié)果滿(mǎn)意,一定程度上表明“分站式”HCR是一種可行的、安全的治療方式,符合我國(guó)國(guó)情,有可能作為冠狀動(dòng)脈多支病變尤其合并高危因素者的治療策略之一,但其尚處于初步發(fā)展階段,需要更多的病例積累,加強(qiáng)長(zhǎng)期隨訪觀察,并需與常規(guī)CABG或常規(guī)PCI進(jìn)行隨機(jī)對(duì)照臨床研究,進(jìn)一步評(píng)價(jià)其臨床效果。

        Objective: To observe the midterm outcomes of “2-staged” hybrid coronary revascularization (HCR) for treating the patients with multi-vessel coronary artery disease (CAD) and to evaluate the feasibility, safety and efficacy of “2-staged”HCR.Methods: A total of 73 relevant patients received elective “2-staged” HCR in our hospital from 2012-01 to 2014-06 were studied. There were 50 (68.5%) male and 23 (31.5%) female at the age of (61.1±10.7) years and all patients had multi coronary artery lesions including left anterior descending (LAD) artery. The key points of “2-staged” HCR wereas follows: double-chamber intubation with general anesthesia, small incision between 4-5 ribs of left front thorax, take left internal mammary artery (LIMA) by direct view and make anastomosis of LIMA and LAD with heartbeat. At (3-5) days postminimally invasive direct coronary artery bypass (MIDCAB), coronary angiography (CAG) was conducted to confirm that LIMA-LAD bypass vessel was unobstructed; then percutaneous coronary intervention (PCI) was performed in non-LAD coronary artery for stent implantation. Post-operative echocardiography, chest X-ray and ECG were examined in each year; coronary CTA or CAG would be taken if the patients with myocardial ischemia.Results: All patients finished “2-staged” HCR smoothly and no operative death occurred. The average surgical time was (152.9±43.8) min and (2.6±0.5) coronary branches were treated, total post-operative drainage volume was (558.6±441.3) ml, red blood cell transfusion was (0.8±1.9) U, mechanical ventilation time was (10.5±13.0) h. The interval between MIDCAB and PCI was (5.3±2) days and (1.6±0.7) stents was implanted. During post-operative follow-up period, there 1 (1.4%) patient died, 3 (4.1%) with recurrent myocardial ischemia, 1 (1.4%) with in-stent restenosis and received PCI again, 4 (5.5%) with MACCE.Conclusion: “2-staged” HCR is a safe and feasible operation with satisfactory peri-operative and mid-term outcomes; it is suitable for the patients with multi-vessel CAD including severe LAD lesions.

        結(jié)論:“分站式”HCR是一種安全,有效的手術(shù)方式,其圍手術(shù)期和中期隨訪結(jié)果滿(mǎn)意,“分站式”HCR適合于左前降支嚴(yán)重病變無(wú)法接受PCI而右冠狀動(dòng)脈主干和(或)回旋支等非左前降支病變可以進(jìn)行PCI的冠狀動(dòng)脈多支病變患者。
