硬膜外阻滯對(duì)老年腹腔鏡胃癌根治術(shù)患者圍術(shù)期Th1/Th2平衡的影響

目的 研究硬膜外阻滯對(duì)腹腔鏡胃癌根治術(shù)老年患者圍術(shù)期Th1/Th2平衡的影響。方法 對(duì)60例擇期行腹腔鏡胃癌根治術(shù)老年患者采用隨機(jī)數(shù)字表法分為硬膜外阻滯組(Ⅰ組)和對(duì)照組(Ⅱ組),每組30例。麻醉誘導(dǎo)前行T8~9間隙行硬膜外穿刺置管。Ⅰ組切皮前約30min時(shí)硬膜外腔注射0.25%羅哌卡因+0.04mg/ml布托啡諾混合液10ml。Ⅱ組給予等量0.9%氯化鈉溶液。兩組患者均在術(shù)畢硬膜外腔注射0.25%羅哌卡因+0.04mg/ml布托啡諾混合液10ml后接PCEA泵。于硬膜外穿刺前(T0)、術(shù)前(T1)、術(shù)畢(T2)、術(shù)后24h(T3)和72 h(T4)抽取外周血,采用放射免疫法檢測(cè)血漿皮質(zhì)醇(Cor)水平,采用ELISA法測(cè)定血漿干擾素(IFN)-γ、IL-4水平。結(jié)果 與T0時(shí)比較,Ⅱ組T2、T3時(shí)Cor水平升高,Ⅰ組僅在T2時(shí)升高(P<0.05)。與Ⅱ組比較,Ⅰ組T2、T3時(shí)Cor水平明顯降低(P<0.05)。與T0比較,兩組T2、T3、T4時(shí)IFN-γ水平明顯上升(P<0.05)。Ⅰ組T2、T3時(shí)IFN-γ水平明顯高于Ⅱ組(P<0.05)。Ⅰ組T3時(shí)IL-4水平低于T0(P<0.05)。與Ⅱ組比較,Ⅰ組T4時(shí)IL-4水平明顯降低(P<0.05)。與T0時(shí)比較,兩組T2、T3、T4時(shí)IFN-γ/IL-4均升高(P<0.05);Ⅰ組T3時(shí)IFN-γ/IL-4明顯高于Ⅱ組(P<0.05)。結(jié)論 硬膜外阻滯能減輕腹腔鏡胃癌根治術(shù)老年患者手術(shù)后Th1/Th2平衡的漂移程度,改善機(jī)體手術(shù)創(chuàng)傷后免疫受抑狀態(tài)。
硬膜外麻醉 全身 干擾素-γ 白細(xì)胞介素-4 老年患者
腫瘤患者術(shù)后免疫系統(tǒng)功能受到抑制,除與術(shù)前自身免疫功能低下有關(guān)外,還受到手術(shù)創(chuàng)傷引起的強(qiáng)烈應(yīng)激反應(yīng)的影響。手術(shù)創(chuàng)傷后T淋巴細(xì)胞激活,T輔助淋巴細(xì)胞(Th)細(xì)胞功能狀態(tài)發(fā)生改變(特別是Th1和Th2),機(jī)體細(xì)胞免疫功能受到抑制[1]。干擾素-γ(IFN-γ)和IL-4分別是Th1和Th2效應(yīng)細(xì)胞具有代表性的細(xì)胞因子,IFN-γ/IL-4比值變化間接反應(yīng)Th1/Th2應(yīng)答模式的漂移[2-3]。筆者通過(guò)測(cè)定硬膜外阻滯對(duì)全麻下腹腔鏡胃癌根治術(shù)老年患者圍術(shù)期血漿皮質(zhì)醇(Cor)、IFN-γ、IL-4水平的變化,評(píng)價(jià)其對(duì)腫瘤患者術(shù)后Th1/Th2平衡的影響,為臨床麻醉提供參考,現(xiàn)報(bào)道如下。
1.1 對(duì)象 本研究已獲本院倫理委員會(huì)批準(zhǔn),并與患者及家屬簽署知情同意書。選取本院2012年8月至2013年7月?lián)衿谛形赴└涡g(shù)患者60例,年齡65~75歲,平均(70.8±5.1)歲;ASAⅠ或Ⅱ級(jí)。按隨機(jī)數(shù)字表法分為硬膜外阻滯組(Ⅰ組)和對(duì)照組(Ⅱ組),每組30例。兩組患者性別、年齡、身高、體重、BMI、手術(shù)時(shí)間及出血量比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(均P>0.05),詳見(jiàn)表1。

1.2 方法 兩組患者術(shù)前禁食10h以上,禁飲4h,無(wú)術(shù)前用藥。患者入手術(shù)室后開放上肢靜脈,16G穿刺輸液,給予乳酸鈉林格液500ml靜脈滴注(20~25min滴完),于T8~9間隙行硬膜外穿刺并置管,左橈動(dòng)脈穿刺置管監(jiān)測(cè)有創(chuàng)血壓,連接腦電雙頻指數(shù)(BIS)監(jiān)測(cè)儀(美國(guó)Aspect Medical Systems公司)監(jiān)測(cè)BIS。麻醉誘導(dǎo):依次靜脈注射咪達(dá)唑侖0.03 mg/kg、芬太尼3.0~4.0μg/kg、丙泊酚1.0~1.5mg/kg、羅庫(kù)溴銨0.9 mg/kg;氣管插管后行機(jī)械通氣,調(diào)節(jié)潮氣量8~10 ml/kg,呼吸頻率12~14次/ min,吸呼比1∶2,維持呼氣末二氧化碳分壓(PETCO2)于35~40mmHg,調(diào)整氧流量2L/min,BIS值維持于40~60。麻醉維持:靜脈輸注瑞芬太尼0.15~0.25μg/(kg·min)及靶控輸注丙泊酚(血漿靶濃度2.5~3.0μg/ml),間斷追加羅庫(kù)溴銨維持肌松。Ⅰ組切皮前30min時(shí)硬膜外腔注射0.25%羅哌卡因(瑞典AstraZeneca AB公司)+0.04mg/ml布托啡諾(江蘇恒瑞醫(yī)藥公司)混合液10ml。Ⅱ組切皮前30min時(shí)硬膜外腔注射等量0.9%氯化鈉溶液。兩組患者均在手術(shù)結(jié)束前硬膜外腔注射上述混合液10ml后接PCEA泵(珠海福尼亞醫(yī)療設(shè)備有限公司),行自控硬膜外鎮(zhèn)痛(PCEA):藥物為羅哌卡因200mg+4mg布托啡諾,用0.9%氯化鈉溶液稀釋為100ml,輸注速率2ml/h,劑量0.5ml,鎖定時(shí)間15min。術(shù)后鎮(zhèn)痛效果不佳時(shí)硬膜外給予0.25%羅哌卡因+0.04mg/ml布托啡諾混合液4~7ml,維持視覺(jué)模擬評(píng)分(VAS)≤3分。
1.3 監(jiān)測(cè)指標(biāo) 采用左側(cè)橈動(dòng)脈穿刺有創(chuàng)測(cè)壓,維持患者術(shù)中血流動(dòng)力學(xué)穩(wěn)定。分別于麻醉前(T0)、術(shù)前(T1)、術(shù)畢(T2)、術(shù)后24h(T3)、72 h(T4)5個(gè)時(shí)點(diǎn)抽取外周靜脈血3ml,離心法分離血漿保存于-70℃冰箱待測(cè)。采用ELISA法測(cè)定血漿IL-4和IFN-γ水平(試劑盒購(gòu)自杭州聯(lián)科生物有限公司);采用放射免疫法檢測(cè)血漿Cor水平(試劑盒購(gòu)自北京華英生物科技研究所)。術(shù)后1、4、24、48h隨訪患者,記錄VAS評(píng)分。
1.4 統(tǒng)計(jì)學(xué)處理 采用SPSS 17.0統(tǒng)計(jì)軟件。計(jì)量資料以表示,組間比較采用獨(dú)立樣本t檢驗(yàn),組內(nèi)不同時(shí)間點(diǎn)的比較采用配對(duì)t檢驗(yàn)。
與T0時(shí)比較,Ⅱ組T2、T3時(shí)Cor水平升高,Ⅰ組僅在T2時(shí)明顯升高;與Ⅱ組比較,Ⅰ組T2、T3時(shí)Cor水平明顯降低。與T0時(shí)比較,兩組T2、T3、T4時(shí)IFN-γ水平明顯上升;Ⅰ組T2、T3時(shí)IFN-γ水平明顯高于Ⅱ組。Ⅰ組T3時(shí)IL-4水平明顯低于T0;與Ⅱ組比較,Ⅰ組T4時(shí)IL-4水平明顯降低。與T0時(shí)比較,兩組T2、T3、T4時(shí)IFN-γ/IL-4均明顯升高;Ⅰ組T3時(shí)IFN-γ/IL-4明顯高于Ⅱ組。兩組患者術(shù)后鎮(zhèn)痛效果均較滿意,各時(shí)點(diǎn)VAS分值差異均無(wú)統(tǒng)計(jì)學(xué)意義(均P>0.05),詳見(jiàn)表2、3。
腫瘤患者術(shù)前常存在不同程度的免疫功能紊亂,手術(shù)創(chuàng)傷、術(shù)后疼痛以及臨床常用的阿片類藥物均可抑制機(jī)體免疫功能[4]。腫瘤患者免疫功能低下可引起術(shù)后感染,殘余腫瘤細(xì)胞生長(zhǎng)、擴(kuò)散,因此術(shù)后盡早恢復(fù)免疫功能可改善患者預(yù)后。T細(xì)胞是細(xì)胞免疫主要的效應(yīng)細(xì)胞之一。由Th1細(xì)胞分泌的細(xì)胞因子如IL-2、IL-12、IFN-γ和TNF-α等在抗腫瘤免疫中發(fā)揮重要作用,Th1細(xì)胞可使機(jī)體處于良好抗腫瘤狀態(tài)。Th2細(xì)胞可產(chǎn)生IL-4、IL-5等細(xì)胞因子,主要功能為促進(jìn)體液免疫應(yīng)答和B細(xì)胞的發(fā)育。惡性腫瘤患者免疫功能低下,手術(shù)創(chuàng)傷、麻醉及術(shù)后疼痛等因素均可使機(jī)體免疫功能,尤其是細(xì)胞免疫功能進(jìn)一步降低。手術(shù)創(chuàng)傷后T淋巴細(xì)胞激活,Th細(xì)胞,特別是Th1和Th2細(xì)胞免疫功能狀態(tài)發(fā)生轉(zhuǎn)變,Th1/Th2平衡向Th2型免疫反應(yīng)漂移,T細(xì)胞功能受到抑制。IFN-γ和IL-4分別是Th1和Th2效應(yīng)細(xì)胞具有代表性的細(xì)胞因子,IFN-γ/IL-4比值變化間接反映Th1/Th2應(yīng)答模式的漂移。

硬膜外阻滯旨在通過(guò)給藥時(shí)機(jī)以防止外周和中樞神經(jīng)的敏感化,達(dá)到減少或消除傷害性刺激引起的疼痛。硬膜外阻滯不僅能更好地抑制上腹部手術(shù)患者術(shù)后應(yīng)激反應(yīng),減少血漿中Cor、CRP和促腎上腺皮質(zhì)激素(ACTH)的釋放[5],而且能降低全麻下腹腔鏡胃癌根治術(shù)老年患者術(shù)后認(rèn)知功能障礙的發(fā)生。所以,硬膜外阻滯能預(yù)防應(yīng)激反應(yīng)及促炎癥介質(zhì)的升高,而手術(shù)創(chuàng)傷一旦引起了應(yīng)激反應(yīng)[6]及促炎介質(zhì)釋放網(wǎng)絡(luò)激活,級(jí)聯(lián)反應(yīng)形成,再使用硬膜外阻滯也不能完全抑制應(yīng)激反應(yīng)及促炎介質(zhì)的升高,這與國(guó)外的研究基本一致[7-8]。本研究發(fā)現(xiàn),使用硬膜外阻滯后,患者IFN-γ和IFN-γ/ IL-4比值升高,IL-4降低,說(shuō)明患者術(shù)后Th1/Th2平衡向Th2型免疫反應(yīng)的漂移程度減輕。
創(chuàng)傷應(yīng)激后Th1/Th2平衡向Th2型免疫反應(yīng)漂移程度與大量糖皮質(zhì)激素的釋放有關(guān),糖皮質(zhì)激素能促進(jìn)Th2細(xì)胞合成IL-4,同時(shí)抑制Th1細(xì)胞功能,糖皮質(zhì)激素還可刺激Th1細(xì)胞核轉(zhuǎn)錄抑制因子I-KB合成增加,I-KB與核因子NF-KB結(jié)合形成復(fù)合物,使NF-KB失活,從而阻止細(xì)胞炎癥因子的基因轉(zhuǎn)錄。另外,糖皮質(zhì)激素還可直接與NF-KB結(jié)合而影響炎癥性細(xì)胞因子的基因轉(zhuǎn)錄。Wada等[9]研究顯示,不同的麻醉藥物和方法通過(guò)影響外科應(yīng)激的程度可以減輕Th1向Th2的漂移。本研究結(jié)果也表明,硬膜外阻滯可以減輕腹腔鏡胃癌根治術(shù)老年患者Cor的升高,減輕外科應(yīng)激的程度。
綜上所述,硬膜外阻滯可以減少腹腔鏡胃癌根治術(shù)老年患者手術(shù)后機(jī)體應(yīng)激激素Cor的分泌,減輕術(shù)后Th1/Th2平衡的漂移程度,改善機(jī)體手術(shù)創(chuàng)傷后T細(xì)胞免疫功能的受抑狀態(tài)。

【 Abstract】 Objective To determine Th1/Th2balance in elderly patients undergoing laparoscopic radical operation for gastric cancer under general anesthesia plus epidural block. Methods Sixty ASA physical I or II patients aged 65-75 y scheduled for elective laparoscopic radical operation for gastric cancer under general anesthesia,were randomly divided into epidural block group(group I,n=30)and control group(group II,n=30).The epidural catheter was placed at T8-9interspace before induction of anesthesia.In group I,10 ml of mixture of 0.25%ropivacaine and 0.04mg/ml butorphanol was injected via the epidural catheter at 30 min before skin incision;the same volume of normal saline was given in group II.The mixture 10 ml 0.25%ropivacaine and 0.04mg/ml butorphanol was administered via the epidural catheter at 10 min before the end of operation followed by patient-controlled epidural analgesia in both groups.Plasma cortisol levels were detected by radioimmunoassay,interferon-γ (INF-γ)and interleukin-4(IL-4)were measured by ELISA before induction of anesthesia(T0),at the begin of operation(T1),at the end of operation(T2),at 24h(T3)and 72h(T4)after operation. Results Compared with T0,plasma cortisol levels were higher at T2and T3in groupⅡand at T3 in groupⅠ (P<0.05).Cortisol levels at T2and T3were lower in groupⅠthan those in groupⅡ (P<0.05).IL-4 level in group I was lower at T3than that at T0(P<0.05).IL-4 at T4was lower in groupⅠthan that in groupⅡ (P<0.05). INF-γ in groupⅠwas higher at T2than that at T0(P<0.05),which was higher at T3than that of groupⅡ (P<0.05).The ratio of IFN-γ/IL-4 in both groups was significantly higher at T2,T3and T4than that at T0,and the ratio of IFN-γ/IL-4 at T3in group I was higher than that in group II(P<0.05). Conclusion Epidural block may alleviate immunological suppression in elderly patients undergoing gastric radical surgery under general anesthesia.
