Abstract

Introduction

Common clinical cardiovascular diseases include heart failure,coronary atherosclerotic heart disease such as angina pectoris and myocardial infarction,arrhythmias such as tachycardia,premature beats and atrial fibrillation,hypertension,cardiomyopathy,myocarditis,valvular disease,etc.The morbidity and mortality of cardiovascular diseases in China are increasing year by year,the age of onset is earlier,and the burden of disease treatment is increasing.At present,cardiovascular disease,as a chronic disease,is mainly controlled by long-term use of western medicine in clinical practice.At the initial stage of the drug medication,the drugs can control the disease.After 1 to 2 years,it is necessary to replace or combine with the medicine for further medication.For patients with repeated onset and even condition aggravation,a hospitalization is required.After discharge,4 to 5 oral drugs are used in combination,but the condition still cannot be effectively controlled,and even liver and kidney metabolism disorders and functional decline occur.Clinical laboratory examinations have found that many patients have abnormalities in other indicators such as creatinine,urea nitrogen,and uric acid.These patients switched to Traditional Chinese medicine treatment and achieved remarkable outcomes.The following is a comparison before and after the application of western medicine and traditional Chinese medicine in 5 typical cases.

Case Data

Case 1:Patient:Yang XX,Female,68 years old,Date of consultation:2012-3-13

Main complaint:Paroxysmal palpitation,chest tightness,and suffocation for 12 years,worsening for 3 days.

History of present illness:12 years ago,the patient had persistent chest pain,palpitation,chest tightness,suffocation,nausea and vomiting,which lasted 1 hour.She went to Emergency Department of the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine and diagnosed as coronary heart disease by coronary angiography,and 1 stent was inserted （specific condition undermined）.At postoperative 1 year,she had regular oral administration of “Plavix”,“Aspirin”and other drugs and reported that chest tightness,pain,suffocation and other symptoms did not recur.From 2008 to March 7,2010,she appeared palpitation,chest tightness,and suffocation after the exercise,each lasting about 1 hour.After resting or taking “Aspirin,Diovan,Plendil”,the above symptoms were gradually alleviated,accounting for 1-5 attacks a week.On March 11,2010,she appeared an aggravation of palpitation,chest tightness,and suffocation compared to before,and occasional chest pain,accompanied by back pain and discomfort,which were induced by walking about 10 meters.She was placed in the lying position with the high pillow at night,and administered the above drugs,but the alleviation was not obvious.Again,she went to the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine and received coronary angiography on March 13,2010.The results showed three-vessel lesions,and 5 stents were inserted.After the operation,she continued to take the aforementioned medications.Chest pain and chest tightness were significantly improved,but there was still heartbeat,palpitation,throbbing and restlessness,which were improved after taking “Betaloc Sustained-Release Tablets”.Liver function and kidney function were detected,blood lipids,creatinine,urea nitrogen,and uric acid abnormalities were found.Palpitations,palpitation,chest tightness and discomfort worsened 3 days ago,the patient came to our clinic for treatment.

Past history:Diabetes history for 14 years,oral administration of “Glucobay” and “NovoRapid”,fasting blood glucose was about 6 mmol/l; history of hypertension for 3 years,maximum 150/100 mmHg,oral administration of “Plendil” and “Diovan”.Normal blood pressure was controlled at 130/80 mmHg; history of dyslipidemia for 3 years,total cholesterol was measured at 6.98 mmol/I,no regular lipid-lowering therapy was performed; coronary heart disease history for 12 years,coronary interventional therapy was performed 12 years ago and 2 years ago respectively.

Case 2:Patient:Wang XX,Female,52 years old,Date of consultation:2014-9-5

Main complaint:paroxysmal palpitation,heartbeat for more than 10 months,worsening for 1 week.

History of present illness:The patient was treated with right thyroid lobectomy for “right lobe papillary carcinoma of the thyroid” at Qianfoshan Hospital in Shandong Province 10 months ago （unknown details）,and took 100 ug qd of Euthyrox after the operation.The patient was discharged in a stable condition.After 2 weeks of discharge,due to paroxysmal palpitation and heartbeat,she went to the Endocrinology Department of Qianfoshan Hospital in Shandong Province for treatment.The dosage of Euthyrox was adjusted to 150 ug po,qd.Symptoms eased,but they still appeared intermittently.One week ago,the patient developed palpitation,heartbeat,heart throbbing,coughing,and unable to cough up phlegm after cold,and chills,cold limbs,and multiple swollen lymph nodes in the neck and underarms with pain.No headache,dizziness,chest tightness,or chest pain,she was admitted to the Department of Cardiology,Qianfoshan Hospital,Shandong Province.The electrocardiogram showed tachycardia and premature atrial contractions.she was given Betaloc 25mg po bid,but the symptoms were not relieved,so she came to our hospital for treatment.

Past history:10 years of history of hypertension,selfadministered “Beijing Antihypertensive 0” treatment,blood pressure is unstable.

Case 3:Patient:Li X,Female,65 years old,Date of consultation:2018-8-21

Main complaint:Heart palpitations worsened for 1 week in recent 2 months.

History of present illness:2 months ago,the patient developed precordial discomfort after catching a cold,and felt her heartbeat and throbbing restlessly.After taking the cold medicine （unknown details）,the cold symptoms disappeared and the precordial discomfort still persisted.The patient went to the community service center near home for check.The electrocardiogram showed:‖,V5,V6 slightly ST-T low signal.After traditional Chinese medicine （unknown details） treatment,she developed stomach discomfort,abdominal distension,anorexia,diarrhea,loose stools,and failure to complete the grain,3 to 5 times a day,then the medication was stopped.She went to Qianfoshan Hospital in Shandong Province for an examination.The echocardiogram showed that the heart structure and blood flow were not detected and obvious abnormality was observed.Thyroid ultrasound （physical examination number 1808020026） showed multiple thyroid nodules,multiple lymph nodes on both sides of the neck,after partial resection of the left thyroid lobe.The doctor prescribed Betaloc 25 mg treatment.There was still discomfort in the precordial area.One week ago,the palpitation and throbbing aggravated,accompanied by chest tightness,and pain on the left side of the back.She went to Qianfoshan Hospital of Shandong Province for examination.Holter showed that there were occasional supraventricular premature beats （visible in dose s and short bursts）,and ST-T changes.So she went to our clinic for treatment.

Past history:history of hypertension,self-administration of Norvasc,blood pressure stabilized at 120/60 mmHg;resection of the left page of the thyroid cyst,history of chronic gastritis.

Case 4:Patient:Du XX,Female,81 years old,Date of consultation:2017-06-06

Main complaint:paroxysmal palpitation,heartbeat for more than 5 months,aggravated with chest tightness for 1 week.

History of present illness:5 months ago,there was paroxysmal palpitation and heartbeat for no obvious reason,self-administered 10 Danshen dripping pills,3 times a day,the effect was not good,1 week ago,palpitation,heart palpitations,alarm and restlessness increased.At the same time,symptoms such as chest tightness,shortness of breath,dizziness,less sleep and more dreams,nightmares,forgetfulness and other symptoms occurred.So she came to our hospital for treatment.

Past history:A history of “atrial fibrillation” for 7 years,long-term oral “Metoprolol” treatment,for continuous atrial fibrillation heart rhythm.A 7-year history of“hypertension”,the highest systolic blood pressure was about 180 mmHg.“Nifedipine Sustained Release Tablets”was given to lower the blood pressure,the blood pressure stabilized at about 140/90 mmHg.A history of “coronary heart disease” for 3-4 years.

Case 5:Patient:Li XX,Female,60 years old,Date of consultation:2017-11-12

Main complaint:paroxysmal heartbeat,palpitation worsened for 3 days in 1 year.

History of present illness:In November 2016,the patient felt discomfort in the precordial area,paroxysmal heartbeat,and palpitation.After self-administering Danshen dripping pills,the symptoms were improved,but she still had seizures after fatigue and mood fluctuation.Due to discomfort in the pre-heart area exacerbated on October 19,2017,she went to the Shandong Provincial Police General Hospital.The examination showed:Thyroid ultrasound （ultrasound number:00333287）showed:Hashimoto’s thyroiditis.The five-item tests of thyroid function showed:anti-thyroglobulin antibody 245.63 IU/ml,anti-thyroid peroxidase antibody 74.15 IU/ml.The final diagnosis was:hypothyroidism.The specific treatment was unknown.3 days ago,she felt heartbeat,palpitation worsened,chest tightness,chest pain,and pain like acupuncture.When the pain was severe,it can radiate to the neck,ears,and mouth.The effect of taking Danshen dripping pills and isosorbide was not good.She went to Shandong Provincial Police General Hospital and the electrocardiogram showed:arrhythmia and occasional ventricular premature beats,ST-T slightly low and flat signal.Then she came to our clinic for treatment.

Past history:hyperlipidemia,self-administration of Simvastatin.

Treatment

Case 1

TCM syndrome differentiation:heart stasis and heart deficiency and cold; Treatment:activating blood to remove blood stasis,regulating qi and dredging collaterals,calming the mind,nourishing the heart and calming mind; prescription:Xuefu Zhuyu Decoction[2]combined with Guizhi Gancao Longmu Decoction[3]addition and subtraction;

Efficacy:2012-3-20 follow-up visit:the patient feels heartbeat,palpitation,chest tightness and stinging episodes are reduced,sleep is improved,nightmares are reduced,terrors are reduced,anxiety and upset are improved.The constipation is relieved.The effect is obvious,the medication is not changed and continue.

2012-4-10 follow-up visit:The patient continued to take the medication for 21 dose s,and the symptoms disappeared without any discomfort.Good appetite,good sleep,regular urine and stool.Since taking Chinese medicine,western medicine has not stopped.At present,blood pressure is stable at about 125/75 mmHg,fasting blood glucose is stable at 5-6 mmol/L,2 hours after meal stable at 8～9 mmol/l,creatinine and urea nitrogen are restored normal range.

The patient complained that she had been treated with a large number of western medicines since the onset of the disease,but her condition continued to recur,and her body had many discomfort.Since taking the abovementioned Chinese medicine,the symptoms have gradually disappeared and the biochemical indicators have been stable.She required Chinese medicine pills and powder for further administration.According to the patient’s condition,the medicine is prepared into water pills,6 g each time,three times a day,and taken after meals.Further consolidate treatment.

2012-9-6 follow-up visit:The patient did not feel any discomfort after taking water pills for 3 months.Afterwards,the amount of western medicine was gradually reduced,and she continued to take Chinese medicine pills.Since follow-up,she has not had any discomfort.

Case 2

TCM syndrome differentiation:phlegm fire disturbing the heart; Western medicine diagnosis:arrhythmia,tachycardia,premature atrial contractions; treatment:clearing away heat and resolving phlegm,calming the mind and calming nerves,reconciling Shaoyang;prescription:Huanglian Wendan Decoction[4]addition and subtraction.

Efficacy:2014-9-12 follow-up visit:After taking the patient,the patient felt palpitation,throbbing and restlessness alleviated.Bitter mouth,dry mouth improved,swelling and pain in both ribs disappeared.The lymph nodes in the neck and axilla are reduced compared to the front,and the pain is alleviated.She can expectorate phlegm,yellow phlegm,occasionally white sticky phlegm.Better sleep,regular urine and stool.The medication is not changed,the patient continues taking 7 dose s in decoction as before.

2014-09-20 follow-up visit:The patient only occasionally flusters after taking it,and the throbbing disappears.No bitterness,dry mouth.The lymph nodes in the neck and axilla are further reduced.There is still coughing sputum,which is white sticky sputum.Sleep has been improved,and the urine and stools are adjusted.The tongue is pale red,the fur is white and greasy,and the pulse is slippery.After the patient took the medicine,the evil heat disappeared.The original prescription added or subtracted as appropriate,followed by 7 doses.

2014-9-27:After taking the administration,the palpitation disappears,the appetite is good,the sleep is good,and the urine and stool are regular.The lymph nodes in the neck and bilateral axilla are further reduced,and white phlegm is reduced.The tongue is pale red,the coating is white,and the pulse is slippery.Adjust the prescription to make water pills to take 3 months.After taking the pills for a follow-up visit,the enlarged lymph nodes disappear completely.Afterwards,take Xiaoyao pills intermittently to adjust the balance of the viscera.Afterwards,the ECG re-examination show a normal ECG.

Case 3

Efficacy:2018-8-28 follow-up visit:The patient feels that her symptoms have improved,her heartbeat,throbbing and restlessness have been significantly improved,she is satisfied,no abdominal distension,bowel movements,dry mouth and tongue astringency are alleviated,and she feels bitterness in the mouth and fever in the palms and feet.Analysis:After the onset of exogenous infection,the evil entering Shaoyin causes heart palpitations.After the current medication is used to warm the heart yang,nourish the qi,nourish the nerves,and sweat to relieve the surface and remove the evil,the evils go out from the inside and out,enter the Shaoyang,Taiyang and Yangming,so the mouth is bitter and the mouth is dry,astringent,fever of the hands and feet.Now make the recipe Zhigancao Decoction and Xiao Chai Hu Decoction[6]addition and subtraction.

Case 4

Case 5

TCM syndrome differentiation:heart stasis; Western medicine diagnosis:arrhythmia,paroxysmal ventricular premature beats.Treatment:Soothing the liver,regulating qi,removing blood stasis and removing phlegm;prescription:Xiao Chaihu Decoction[6]combined with Guizhi Licorice Longmu[3]decoction;

Efficacy:2017-11-19 follow-up visit:palpitation,chest tightness,tingling pain relieved,mouth pain relieved,sleep improvement,stool adjustment.The medication is effective and remains for another 7 doses as before.

2017-11-26 follow-up visit:heartbeat and palpitations do not reappear,chest tightness and tingling pain are significantly reduced,slightly bitter mouth in the morning,no dry throat,no two hypochondriac pains,sleep is significantly improved,tongue texture was slightly darker,and the coating was slightly thin and white yellow.Regular urine and stool.Add and subtract the medication as appropriate for additional 7 doses.
