[摘要]目的:觀察比較唇裂及硬腭一期修復(fù)的手術(shù)方法與傳統(tǒng)方法對上頜骨發(fā)育的影響。方法:將30例單側(cè)完全性唇腭裂患兒隨機(jī)分為數(shù)量相等的兩組。觀察組:嬰兒期行唇裂及硬腭一期修復(fù),二期行腭裂修復(fù),隨訪時平均年齡(5.99±0.49)歲;對照組:嬰兒期僅行唇裂修復(fù),二期行腭裂修復(fù),隨訪時平均年齡(6.05±0.54)歲;另15例無先天性唇腭裂的6歲齡兒童作為正常組;三組分別取頭顱側(cè)位片及上牙頜模型,進(jìn)行測量分析。結(jié)果:兩組唇腭裂患兒之間的上頜骨發(fā)育無明顯差異,與正常組比較,均有生長抑制,對照組上頜骨發(fā)育受限更明顯。結(jié)論:唇裂及硬腭一期修復(fù)可以適當(dāng)減輕腭裂手術(shù)對上頜骨發(fā)育的影響,但與傳統(tǒng)治療方法相比尚無明顯差異。

The Preliminary Effect of Simultaneous Cleft Lip and Hard Palate Crack Repair on Maxillary Development

Abstract: Objective ?To observe the effect of one-stage repair of cleft lip and hard palate crack on the development of maxillary compared with traditional methods. Methods Thirty children with unilateral complete cleft lip and palate were randomly divided into two groups. The observation group received one-stage repair of cleft lip and hard palate crack in infancy, and cleft palate repaired at the second stage. The average age ?was (5.99±0.49) years old at follow-up. In the control group, only cleft lip repair was performed in infancy, and cleft palate repair was performed at the second stage. The average age ?was (6.05±0.54) years at follow-up. Another 15 cases of 6 years old children without congenital cleft lip and palate were considered as normal group. The X-ray cephalometry and the upper dentognathic model were taken for measurement and analysis in three groups. Results There was no significant difference in maxillary development between the two groups of children with cleft lip and palate, compared with the normal group, growth inhibition was observed in both groups, and the maxillary development restriction ?was more obvious in the control group. Conclusion ?Cleft lip and hard palate crack one-stage repair can reduced the influence of palatorrhaphy on the development of maxillary, but there was no significant difference from traditional methods.

先天性腭裂疾病必須通過手術(shù)予以修復(fù),腭裂手術(shù)在關(guān)閉腭裂完善腭咽閉合的同時,手術(shù)產(chǎn)生的大面積上腭骨面裸露、纖維組織增生致上頜骨發(fā)育明顯受限,為了減輕這種手術(shù)并發(fā)癥,筆者科室在唇裂手術(shù)同期行犁骨瓣硬腭修復(fù),后期再行軟腭裂修復(fù)。筆者科室前期研究已證明該術(shù)式可行[1],且硬腭的修復(fù)可使上腭裂隙縮窄的更明顯,腭裂修復(fù)時較傳統(tǒng)方法所作松弛切口更少,還可以減少術(shù)后上腭骨面裸露的面積[2]。為進(jìn)一步明確比較該術(shù)式與傳統(tǒng)術(shù)式對上頜骨發(fā)育的影響,筆者對兩組術(shù)式的患兒進(jìn)行隨訪,取頭顱側(cè)位片及上牙頜模型進(jìn)行比較,現(xiàn)將結(jié)果報(bào)道如下。
1 ?資料和方法
1.1 研究對象:30例單側(cè)完全性唇腭裂患兒,其中觀察組15例為在嬰兒期行了唇裂及犁骨瓣硬腭一期修復(fù),1~1.5歲行了腭裂修復(fù),然后進(jìn)行隨訪,隨訪時年齡5~7歲,平均(5.99±0.49)歲,手術(shù)前后未接受正畸及正頜手術(shù)治療;對照組15例為在嬰兒期僅行唇裂修復(fù),1~1.5歲行了腭裂修復(fù),后進(jìn)行隨訪時年齡5~7歲,平均(6.05±0.54)歲,手術(shù)前后也未接受正畸及正頜手術(shù)治療,兩組患者的手術(shù)均由同一位醫(yī)生完成;正常組為15例無先天性唇腭裂的6歲齡學(xué)童,未接受過牙齒正畸治療。
1.2 研究方法:對三組兒童均在相同條件下按標(biāo)準(zhǔn)化條件拍攝頭顱側(cè)位片并取上牙頜模型。頭顱側(cè)位片定點(diǎn)測量∠SNA、∠ANB、N-ANS、ANS-PNS(見圖1)。在牙頜模型上分別測量上頜牙弓長(Ⅰ-Ⅴ)、牙弓前段寬(Ⅲ-Ⅲ)及牙弓后段寬(Ⅴ-Ⅴ)(見圖2),所有數(shù)據(jù)均由筆者測量 3 次取其平均值。
1.3 統(tǒng)計(jì)學(xué)分析:采用統(tǒng)計(jì)軟件對三組均數(shù)兩兩進(jìn)行t檢驗(yàn),P<0.05(雙側(cè))為差別有統(tǒng)計(jì)學(xué)意義。
2 ? 結(jié)果
三組兒童各項(xiàng)指標(biāo)測量值結(jié)果比較,見表1。從所得結(jié)果可以看出在反映上頜骨及牙弓發(fā)育的各項(xiàng)指標(biāo)中,包括上頜骨的矢狀向發(fā)育(∠SNA和∠ANB)、上頜骨的高度發(fā)育(N-ANS)、上頜骨的長度發(fā)育(ANS-PNS)及上頜牙弓長(Ⅰ-Ⅴ)、牙弓前段寬(Ⅲ-Ⅲ)及牙弓后段寬(Ⅴ-Ⅴ)上,觀察組與對照組存在一定差異,對照組各項(xiàng)指標(biāo)更小,但這種差異不明顯(P>0.05);兩組患兒與正常兒童相比較,上述各指標(biāo)均存在明顯差異(P<0.05),且在上頜骨的長度(ANS-PNS)與牙弓的長度(Ⅰ-Ⅴ)及牙弓前段寬(Ⅲ-Ⅲ)的指標(biāo)上,對照組與正常組差異更大(P<0.01)。
3 ?討論
完全性腭裂手術(shù)后上腭術(shù)區(qū)會出現(xiàn)裸露骨面,其修復(fù)是通過肉芽組織再生,即瘢痕組織來完成的。廣泛的瘢痕組織附著于腭骨,瘢痕組織中缺乏彈力纖維,瘢痕收縮所產(chǎn)生的張力使牙槽骨發(fā)育產(chǎn)生了移位,從而縮窄了上頜骨的寬度,上頜骨在各個方向上的生長均會受到抑制,產(chǎn)生畸形;同時術(shù)中對腭骨骨膜的廣泛剝離,導(dǎo)致其發(fā)育營養(yǎng)障礙也是對上頜骨發(fā)育的重要抑制因素[3-5]。Rohit等[6]研究發(fā)現(xiàn)腭裂手術(shù)使上頜骨發(fā)育更短,上頜骨前部的位置更加靠后。Bittencourt等[7]認(rèn)為腭裂術(shù)后患者存在上頜骨橫向發(fā)育不足,且從前向后呈減輕的趨勢。本研究中,兩組患兒在與正常組進(jìn)行各項(xiàng)指標(biāo)比較時,包括上頜骨的長度、高度、矢狀方向的生長及牙弓的長寬,均存在明顯差異(P<0.05),生長發(fā)育受限,這應(yīng)該與腭裂手術(shù)產(chǎn)生的瘢痕組織有關(guān)系。而在上頜骨長度(ANS-PNS)、牙弓長度(Ⅰ-Ⅴ)、牙弓前段寬(Ⅲ-Ⅲ)這幾項(xiàng)指標(biāo)上,對照組與正常組之間差異更加明顯(P<0.01),發(fā)育受限更嚴(yán)重,這可能與兩組患兒在腭裂修復(fù)上采用了不同的術(shù)式有一定關(guān)系。筆者在前期研究中已經(jīng)發(fā)現(xiàn)唇裂硬腭一期修復(fù)的觀察組患兒在行腭裂手術(shù)時,其上腭裂隙較傳統(tǒng)方法修復(fù)的對照組縮窄更明顯,所作松弛切口更少[2],這可以減少術(shù)后上腭硬腭骨面裸露的面積,上腭硬腭區(qū)產(chǎn)生的瘢痕減少,使得瘢痕組織在硬腭區(qū)的束縛力減弱,而硬腭區(qū)對應(yīng)了整個的牙弓前段,所以在與正常組比較時,觀察組牙弓前段寬的縮窄沒有對照組嚴(yán)重,同時瘢痕對于牙弓及上頜骨長度的影響也相對小。當(dāng)然,兩組患兒之間的這種差異尚不明顯,這個可以從觀察組與對照組的各項(xiàng)指標(biāo)對比結(jié)果得出。
唇腭裂患兒的頜骨發(fā)育畸形,除了腭裂手術(shù)的原因之外,也有研究認(rèn)為其和患兒自身的先天性發(fā)育缺陷及唇裂手術(shù)有一定關(guān)系。Bishara[8]研究發(fā)現(xiàn)未行手術(shù)的單側(cè)完全性唇腭裂患者,其上頜存在明顯后縮。Derike等[9]對未接受手術(shù)患者的研究發(fā)現(xiàn)這類患者尖牙區(qū)牙弓寬度縮窄可能是因?yàn)橄忍旎卫奂吧项M骨前部發(fā)育所致。Ye等[10]也認(rèn)為唇腭裂畸形本身會抑制上頜骨發(fā)育。而對于唇裂手術(shù)有研究顯示其對上頜骨的發(fā)育抑制,主要表現(xiàn)在前后向長度的發(fā)育上受到抑制[11],Yoshid等[12]的頭影測量發(fā)現(xiàn)唇裂手術(shù)使上頜骨既有位置后移,也有矢狀方向長度的縮短。有學(xué)者[13]研究中發(fā)現(xiàn)唇裂手術(shù)使患者出現(xiàn)面中部發(fā)育障礙,牙弓的矢狀方向前后段都有不同程度的縮短,前段縮短更嚴(yán)重。上述研究結(jié)果提示本研究中觀察組與對照組存在的發(fā)育畸形也可能與唇裂手術(shù)及患兒自身發(fā)育缺陷有關(guān),而兩組患兒均為單側(cè)完全性唇腭裂,唇裂手術(shù)均由同一醫(yī)生完成[2],應(yīng)該說在先天發(fā)育缺陷及唇裂手術(shù)方面未存在明顯差異,如果是這兩個因素主導(dǎo)的患兒上頜骨發(fā)育畸形,也就解釋了兩組患兒在上頜骨發(fā)育方面無明顯差異的原因。當(dāng)然現(xiàn)在對于哪種因素是影響上頜骨發(fā)育的主導(dǎo)因素尚沒有統(tǒng)一定論,腭裂手術(shù)的影響程度也不是很確定。
頜面部快速生長期為3周~7個月、4~7歲、11~15歲[14],而本課題組最早開展這項(xiàng)手術(shù)在2013年,故三組兒童年齡均選在6歲左右,即第二個生長發(fā)育的高峰時期進(jìn)行對比,這是一個階段性的結(jié)果,隨著患兒的進(jìn)一步發(fā)育,頜骨結(jié)構(gòu)也將出現(xiàn)新的變化,這個將有待于進(jìn)一步的追蹤隨訪。
