血清C反應(yīng)蛋白與前白蛋白比值對(duì)慢性阻塞性肺疾病伴下呼吸道細(xì)菌感染的早期診斷價(jià)值

目的采用ROC曲線評(píng)估血清C-反應(yīng)蛋白(CRP)與前白蛋白(PA)比值(CRP/PA)對(duì)慢性阻塞性肺疾病急性加重(AECOPD)合并下呼吸道細(xì)菌感染的早期診斷價(jià)值。方法選擇2013年8月~2014年4月浙江省立同德醫(yī)院、浙江省立同德醫(yī)院海鹽分院和上海市浦東新區(qū)浦南醫(yī)院呼吸內(nèi)科臨床疑似合并細(xì)菌感染的AECOPD患者105例,于入院治療前及治療后達(dá)穩(wěn)定期時(shí)檢測血清PA和CRP水平,采集痰液送細(xì)菌學(xué)培養(yǎng)。根據(jù)臨床表現(xiàn)和病菌培養(yǎng)結(jié)果將105例AECOPD患者分為重度感染組(19例)、輕度感染組(45例)和非感染組(41例),比較三組患者治療前后血清PA和CRP水平的變化,采用ROC曲線評(píng)估血清CRP/PA比值對(duì)早期診斷AECOPD合并下呼吸道細(xì)菌感染的價(jià)值。結(jié)果治療前,重度感染組血清PA水平顯著低于輕度感染組(P<0.01),輕度感染組顯著低于非感染組(P<0.01);重度感染組血清CRP水平顯著高于輕度感染組(P<0.01),而輕度感染組高于非感染組(P<0.01)。治療后,重度感染組PA水平較治療前顯著升高(P<0.01),三組患者CRP水平均較治療前顯著下降(P<0.01),但三組間CRP水平差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。ROC曲線分析顯示:血清CRP/PA>0.1時(shí),預(yù)測細(xì)菌感染的敏感性、特異性和曲線下面積為93.4%、88.4%、0.881,均高于PA、CRP及PA聯(lián)合CRP(P<0.01),Youden指數(shù)(0.588)高于PA和PA聯(lián)合CRP(P<0.01)。結(jié)論血清CRP/PA比值在早期診斷AECOPD是否合并下呼吸道細(xì)菌感染方面具有較高的敏感度和特異性,有利于指導(dǎo)抗生素的使用。

慢性阻塞性肺疾病急性加重(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)80%是由下呼吸道感染引起,但細(xì)菌與病毒感染臨床難以區(qū)分,另外病原學(xué)檢測的滯后性限制了臨床早期診斷[1]。近年來,血清降鈣素原、C-反應(yīng)蛋白(C-reactive protein,CRP)及髓樣細(xì)胞觸發(fā)受體-1等可作為細(xì)菌感染的生物標(biāo)志物[2-6],但因作用不穩(wěn)定或檢測條件限制而影響了臨床應(yīng)用。本研究對(duì)疑似合并細(xì)菌感染的105例AECOPD患者,檢測血清前白蛋白(prealbumin,PA)和CRP水平,旨在探討血清CRP/PA比值在AECOPD合并下呼吸道細(xì)菌感染早期診斷中的價(jià)值。
1.1 一般資料
選擇2013年8月~2014年4月浙江省立同德醫(yī)院、浙江省立同德醫(yī)院海鹽分院和上海市浦東新區(qū)浦南醫(yī)院呼吸內(nèi)科疑似合并下呼吸道細(xì)菌感染AECOPD[7]患者133例,AECOPD診斷根據(jù)《慢性阻塞性肺疾病診治指南(2013年修訂版)》,并排除其他慢性呼吸系統(tǒng)疾病、入選時(shí)加重期病程已超過5 d、近4周內(nèi)用過抗生素、胸片證實(shí)有肺炎及呼吸系統(tǒng)外存在細(xì)菌感染。133例患者中105例符合入選條件,其中男67例,女38例。所有患者于入院治療前及治療后達(dá)穩(wěn)定期時(shí)檢測血清PA和CRP水平,采集痰液送細(xì)菌學(xué)培養(yǎng),并根據(jù)痰細(xì)菌學(xué)培養(yǎng)結(jié)果將105例AECOPD患者分為細(xì)菌感染組(簡稱為感染組)64例和非感染組41例;再根有無感染性休克或是否機(jī)械通氣(符合一項(xiàng)或以上)將感染組分為重度感染組(19例)和輕度感染組(45例)。
1.2 方法
1.2.1 痰細(xì)菌定量培養(yǎng)患者入院當(dāng)天治療前,漱口后深咳痰的膿性痰送檢,無痰者可采用3%高滲鹽水霧化吸入導(dǎo)痰,痰液留置無菌容器中,2 h內(nèi)送檢,連送3 d。對(duì)涂片鏡檢合格痰標(biāo)本(每低倍視野鱗狀上皮細(xì)胞<10個(gè),多核白細(xì)胞>25個(gè))進(jìn)行痰細(xì)菌定量培養(yǎng)[1]。采用瓊脂巧克力培養(yǎng)基,菌種培養(yǎng)鑒定采用法國梅里埃公司VITEK32自動(dòng)細(xì)菌鑒定儀測定。痰培養(yǎng)為下呼吸道同一潛在病原菌連續(xù)2次及以上,且病原菌濃度>1×107cfu/mL作為COPD下呼吸道細(xì)菌感染的診斷標(biāo)準(zhǔn)[8]。
1.2.2 血清CRP和PA檢測患者入院當(dāng)天治療前及治療后達(dá)穩(wěn)定期時(shí)分別抽取靜脈血2 mL,分離血清,貯存于-20℃待測。PA采用免疫濁度法測定,CRP采用乳膠增強(qiáng)透射免疫比濁法測定,嚴(yán)格按照說明書進(jìn)行操作。
1.3 統(tǒng)計(jì)學(xué)方法
采用統(tǒng)計(jì)軟件SPSS 17.0對(duì)數(shù)據(jù)進(jìn)行分析,計(jì)量資料經(jīng)Kolmogorov-Smirnov檢驗(yàn)為正態(tài)分布以均數(shù)±標(biāo)準(zhǔn)差表示,多組間比較采用ANOVA檢驗(yàn),兩兩比較采用LSD-t檢驗(yàn)。非正態(tài)分布的計(jì)量資料用中位數(shù)(M)及四分位數(shù)間距(Q),采用Kruskal-Wallis H檢驗(yàn)。計(jì)數(shù)資料以率表示,采用χ2檢驗(yàn)。CRP/PA比值對(duì)AECOPD伴下呼吸道細(xì)菌感染的診斷價(jià)值采用受試者工作特征曲線(receiver operating characteristic curve,ROC)分析,陽性預(yù)測值(PPV)、陰性預(yù)測值(NPV)及Youden指數(shù)由計(jì)算得出。以P<0.05為差異有統(tǒng)計(jì)學(xué)意義。
2.1 三組患者一般資料
三組患者在性別、年齡、吸煙史等一般資料比較,差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。見表1。
2.2 三組患者治療前、后血清PA和CRP水平的比較
治療前,重度感染組血清PA水平顯著低于輕度感染組(P<0.01),輕度感染組顯著低于非感染組(P<0.01);重度感染組血清CRP水平顯著高于輕度感染組(P<0.01),而輕度感染組高于非感染組(P<0.01)。治療后,重度感染組PA水平較治療前顯著升高(P<0.01),三組患者CRP水平均較治療前顯著下降(P<0.01),而三組間CRP水平差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。見表2。

2.3 血清CRP/PA比值預(yù)測AECOPD合并下呼吸道細(xì)菌感染的ROC曲線分析
當(dāng)CRP/PA>0.1時(shí),預(yù)測細(xì)菌感染的敏感度、特異性和曲線下面積(AUC)均高于PA、CRP及PA聯(lián)合CRP。見表3、圖1。
COPD是全救高發(fā)病率、高死亡率的疾病[9]。AECOPD常見感染病原體為細(xì)菌或病毒[4],快速鑒別病原體對(duì)于抗生素的使用很重要[10]。目前AECOPD抗生素治療指征[7]為:①呼吸困難加重、痰量增加和膿性痰是3個(gè)必要癥狀;②膿性痰在內(nèi)的2個(gè)必要癥狀;③需要有創(chuàng)或無創(chuàng)機(jī)械通氣治療。但這些指征存在較高的誤診率和漏診率。本研究選擇PA和CRP均屬于常規(guī)檢測,簡便、快速而經(jīng)濟(jì),血清CRP/PA比值對(duì)于早期判斷AECOPD合并下呼吸道細(xì)菌感染具有較高的敏感度和特異性,有利于指導(dǎo)臨床用藥。
血清PA屬于肝細(xì)胞合成的快速轉(zhuǎn)運(yùn)蛋白之一,是一種負(fù)急性時(shí)相蛋白,半衰期短,合成快,也與炎癥呈負(fù)相關(guān)[11,13]。PA作為一種非特異性防御物質(zhì),可以清除有毒代謝產(chǎn)物,并被逐漸消耗[13]。本研究顯示,當(dāng)PA低于界值(269.5 mg/L)時(shí),PA預(yù)測細(xì)菌感染的敏感度為86.2%,但特異性較低為54.8%。CRP由肝臟合成,當(dāng)體內(nèi)有急性炎癥、細(xì)菌感染、組織破壞及惡性腫瘤時(shí)很快升高,而治愈時(shí)很快恢復(fù)正常[14]。本研究表明,AECODP感染組血清CRP水平顯著高于非感染組,經(jīng)抗感染治療達(dá)到穩(wěn)定期后血清CRP水平又迅速下降。當(dāng)血清CRP水平高于18.5 mg/L時(shí),在預(yù)測細(xì)菌感染方面具有較高的PPV(89.1%),Youden指數(shù)較高為0.620,特異性為85.2%。可見,PA或CRP檢測對(duì)于早期識(shí)別AECOPD并發(fā)細(xì)菌感染特異性較低,可出現(xiàn)一定比例的誤診率。

血清CRP/PA比值是指血清CRP水平與血清PA水平的比值。有研究[15]發(fā)現(xiàn),危重患者血清CRP/PA比值與器官功能障礙嚴(yán)重程度呈密切相關(guān)。本研究表明,血清CRP/PA與細(xì)菌感染密切相關(guān)。當(dāng)CRP/PA>0.1時(shí),其預(yù)測細(xì)菌感染的敏感性(93.4%)、特異性(88.4%)和AUC(0.881)均高于PA、CRP及PA聯(lián)合CRP,特異性高于血清前降鈣素的文獻(xiàn)報(bào)道(60.0%)[1],Youden指數(shù)(0.588)高于PA和PA聯(lián)合CRP。血清CRP/PA比值對(duì)AECOPD合并細(xì)菌感染的早期預(yù)測具有更高的敏感度和特異性。
綜上所述,血清CRP/PA比值可作為一種適宜指標(biāo),在早期診斷AECOPD合并下呼吸道細(xì)菌感染方面有較高的敏感度和特異性,有助于指導(dǎo)抗菌藥物的合理使用。

ObjectiveTo explore the value of serum C-reactive protein(CRP)and prealbumin(PA)ratio(CRP/PA)in the early diagnosis of lower respiratory tract bacterial infection in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).MethodsFrom August 2013 to April 2014,in Tongde Hospital of Zhejiang Province,Haiyan Branch of Tongde Hospital of Zhejiang Province and Pu'nan Hospital of Pudong New District,105 AECOPD patients with suspicion of lower respiratory tract bacterial infection were selected,at the admission day before treatment and after the treatment for stability,serum PA and CRP levels were detected,and sputum bacterial cultivation was taken.According to associated clinical manifestations and sputum bacterial cultivation results,These patients were divided into severe infection group(19 cases),mild infection group(45 cases)and non-infection group(41 cases).Receiver operating characteristic curve (ROC)was used to evaluate the value of serum CRP/PA ratio in the early diagnosis of lower respiratory tract bac-terial infection in AECOPD patients.ResultsBefore treatment,serum PA level in mild infection group was significantly lower that in severe infection group(P<0.01),this in mild infection group was significantly lower than that in non-infection group(P<0.01);serum CRP level in severe infection group was higher than that in severe infection group(P<0.01),this in mild infection group was significantly higher than that in non-infection group(P<0.01).After treatment, the PA level in severe infection group significantly increased(P<0.01),serum CPR level significantly decreased than (P<0.01),but CRP in three groups was compared,the difference was not statistically significant(P>0.05).ROC curve analysis showed:when serum CRP/PA>0.1,the sensitivity,specificity and the area under curve(AUC)were 93.4%, 88.4%,0.881 respectively,they were higher than those of PA,CRP and PA tandem/parallel CRP.The Youden index was 0.588,it was higher than those of PA tandem/parallel PA tandem CRP.ConclusionSerum CRP/PA ratio has higher sensitivity and specificity in the early diagnosis of lower respiratory tract bacterial infection in AECOPD patients,it is helpful to guide the administration of antibacterials.
