        經(jīng)大鼠腰骶部鞘內(nèi)置管給藥技術(shù)的研究

        【摘要】 目的:改進(jìn)大鼠經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù),探索更加簡(jiǎn)便、快捷、安全、有效的大鼠鞘內(nèi)給藥的新方法。方法:健康雄性大鼠60只,按隨機(jī)數(shù)字表法分為經(jīng)寰枕膜鞘內(nèi)置管組(C組,n=30)和經(jīng)皮穿刺腰骶部鞘內(nèi)置管組(T組,n=30)。戊巴比妥鈉腹腔麻醉后,C組大鼠接受經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管操作,T組大鼠接受在L6~S1間隙,用20 G穿刺針行經(jīng)皮穿刺腰骶部鞘內(nèi)置管操作。兩組大鼠在操作成功后,均經(jīng)導(dǎo)管注入2%利多卡因10 ?L。觀察并比較兩組大鼠置管成功只數(shù)、癱瘓只數(shù)以及死亡只數(shù)。結(jié)果:與C組比較,T組大鼠鞘內(nèi)置管成功率明顯提高,術(shù)后癱瘓、死亡率明顯減少,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論:經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù)是一項(xiàng)簡(jiǎn)便、快捷、安全、有效的鞘內(nèi)給藥方法。

        【Abstract】 Objective:To explore a simple, safe, fast and effective method of intrathecal application by studying the method of intrathecal catheterization via lumbosacral puncture in rats.Method:60 healthy rats were randomly divided into the intrathecal catheterization through the atlantooccipital membrane group (the group C, n=30)and the intrathecal catheterization through lumbosacral puncture group (the group T, n=30). All the rats in the two groups received abdominal anesthesia by pentobarbital natrium. The group C received intrathecal catheterization by the atlantooccipital membrane. The group T received intrathecal catheterization by 20 G needle in L6-S1 interspace. Then, all the rats in the two groups were administrated 2% lidocaine hydrochloride 10 μL via the catheter. The number of successful catheterization, paralysis and death were compared in the two groups.Result:Compared with the group C, the success rate of catheterization was significantly increased, the incidence of death and paralysis were significantly decreased in the group T, the differences were statistically significant (P<0.05).Conclusion:Intrathecal catheterization by lumbosacral puncture is an effective and safe intrathecal catheterization method for rats undergoing scientific experiments.

        大鼠鞘內(nèi)置管給藥是研究藥物在脊髓水平作用機(jī)制的常用實(shí)驗(yàn)方法,廣泛應(yīng)用于疼痛領(lǐng)域的研究[1-4]。尤其對(duì)于需要在大鼠脊髓腰膨大部位進(jìn)行鞘內(nèi)給藥的實(shí)驗(yàn)研究,鞘內(nèi)置管給藥技術(shù)更是尤為重要。傳統(tǒng)的鞘內(nèi)置管是經(jīng)寰枕膜鞘內(nèi)置管,由于是經(jīng)皮切開(kāi)置管,創(chuàng)傷較大,且PE-10聚乙烯導(dǎo)管要下行經(jīng)脊髓頸曲、胸曲后才能到達(dá)大鼠腰膨大的部位,術(shù)中易損傷脊髓而導(dǎo)致大鼠癱瘓或死亡,并且大鼠接受經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管操作后,術(shù)后需要修養(yǎng)恢復(fù)5~7 d后才能觀察其痛行為反應(yīng)。這種方法周期長(zhǎng),大鼠癱瘓率或死亡率較高[5]。近幾年,隨著鞘內(nèi)給藥技術(shù)的不斷改良,反復(fù)腰骶部直接鞘內(nèi)注射給藥技術(shù)受到大家的關(guān)注,但是由于注藥部位在腰骶部,無(wú)法滿足在胸段給藥的實(shí)驗(yàn)要求。而經(jīng)皮穿刺腰骶部鞘內(nèi)置管給藥技術(shù)的出現(xiàn)解決了這一難題。因此,本研究通過(guò)將經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù)與傳統(tǒng)的經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管技術(shù)進(jìn)行對(duì)比,旨在探討該方法的可行性與優(yōu)越性。

        1 材料與方法

        1.1 實(shí)驗(yàn)動(dòng)物與分組 成年雄性SD大鼠60只,體重230~260 g,由山西醫(yī)科大學(xué)實(shí)驗(yàn)動(dòng)物中心提供,按隨機(jī)數(shù)字表法分為兩組:經(jīng)皮穿刺腰骶部鞘內(nèi)置管組(T組)和經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管組(C組),每組30只。實(shí)驗(yàn)期間,將大鼠置于溫度20~24 ℃、晝夜交替、自由攝食食物與水的環(huán)境中。

        1.2 實(shí)驗(yàn)方法 兩組大鼠均采用戊巴比妥鈉40 mg/kg腹腔內(nèi)注射麻醉,隨后進(jìn)行鞘內(nèi)置管操作,C組采用經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管術(shù),T組采用經(jīng)皮穿刺腰骶部鞘內(nèi)置管術(shù),鞘內(nèi)置管成功后,在體外留置導(dǎo)管1~2 cm,并用消毒的不銹鋼針堵塞PE-10聚乙烯導(dǎo)管外口,以防腦脊液外溢。待大鼠快要清醒,用10 ?L微量注射器連接留置在大鼠體外的導(dǎo)管,兩組大鼠均給予10 ?L 2%利多卡因注射液以便術(shù)后觀察穿刺置管成功率。注藥成功后,留置導(dǎo)管1~2 min,隨后拔出導(dǎo)管。endprint

        1.2.1 經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管 采用傳統(tǒng)的Yaksh等[6]的手術(shù)方法,C組大鼠麻醉后至于實(shí)驗(yàn)臺(tái)上,頸下放置直徑約為2~3 cm圓柱狀物體使枕骨大孔與寰椎之間的間隙充分張開(kāi)便于插管操作。常規(guī)剪毛并用碘伏消毒手術(shù)范圍,依次切開(kāi)皮膚,皮下筋膜,鈍性分離肌肉直至暴露枕骨大孔,用針尖輕輕挑開(kāi)寰枕膜,此時(shí)可見(jiàn)清亮的腦脊液溢出,隨后將充滿生理鹽水的PE-10聚乙烯導(dǎo)管直視下,緩慢插入蛛網(wǎng)膜下腔約7.5 cm,使其尖端位于腰膨大處[7]。

        1.2.2 經(jīng)皮穿刺腰骶部鞘內(nèi)置管 借鑒Li等[8]介紹的方法并加以改進(jìn)。T組大鼠麻醉后置于實(shí)驗(yàn)臺(tái)上,腰骶部下方放置直徑約為3~5 cm圓柱狀物體,使L6~S1之間的間隙充分張開(kāi)便于穿刺操作。常規(guī)剪毛并用碘伏消毒穿刺區(qū)域皮膚,鋪無(wú)菌洞巾,實(shí)驗(yàn)者用拇指和中指分別觸摸大鼠雙側(cè)髂嵴并向兩側(cè)繃緊皮膚,食指于兩側(cè)髂嵴連線同一水平觸摸大鼠脊椎,觸及的最高點(diǎn)即為L(zhǎng)6棘突,在L6~S1間隙采用20 G(外徑0.90 mm,針長(zhǎng)30 mm)穿刺針(穿刺針針尖斜面朝向頭側(cè))由上向下垂直刺入皮膚、皮下肌層,當(dāng)出現(xiàn)明顯的落空感或是阻力消失感,證明針尖已經(jīng)突破黃韌帶和蛛網(wǎng)膜進(jìn)入蛛網(wǎng)膜下腔,此時(shí)將穿刺針稍稍向尾側(cè)傾斜約30°,繼續(xù)向前推進(jìn)1 mm左右即可。隨后將充滿生理鹽水的PE-10聚乙烯導(dǎo)管從穿刺針內(nèi)部緩慢插入約4 cm左右,即到達(dá)腰膨大部位,導(dǎo)管插入過(guò)程中可出現(xiàn)腦脊液流出、動(dòng)物甩尾等顯著特征以充分證明導(dǎo)管置入成功。

        1.3 觀察指標(biāo)

        1.3.1 置管成功率 兩組大鼠術(shù)后清醒后,采用觀察運(yùn)動(dòng)法和鉗夾縮足反應(yīng)法,觀察大鼠蛛網(wǎng)膜下腔阻滯情況。并記錄清醒后雙下肢麻痹,鉗夾不產(chǎn)生縮足反應(yīng),并且于2%利多卡因藥效退去后可完全恢復(fù)正常肢體活動(dòng)能力的大鼠只數(shù),以此判斷蛛網(wǎng)膜下腔置管的成功率。

        1.3.2 并發(fā)癥 兩組大鼠蛛網(wǎng)膜下腔阻滯作用消退后,持續(xù)觀察7 d,記錄術(shù)后兩組大鼠發(fā)生癱瘓只數(shù)以及術(shù)后死亡只數(shù),并進(jìn)行比較。

        1.4 統(tǒng)計(jì)學(xué)處理 采用SPSS 20.0統(tǒng)計(jì)學(xué)軟件對(duì)數(shù)據(jù)進(jìn)行處理,計(jì)數(shù)資料的比較采用 字2檢驗(yàn),以P<0.05為差異有統(tǒng)計(jì)學(xué)意義。

        2 結(jié)果

        2.1 兩組置管成功率比較 T組置管成功率為96.7%(29/30),明顯高于C組的60.0%(18/30),差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。C組8只大鼠,T組19只大鼠術(shù)后清醒后出現(xiàn)雙后肢麻痹,用鑷子鉗夾雙后足均不引起縮足反應(yīng)。大約3 h左右雙下肢肌力慢慢恢復(fù),活動(dòng)自如,未發(fā)現(xiàn)任何肢體運(yùn)動(dòng)障礙。

        2.2 兩組并發(fā)癥比較 與C組相比,T組大鼠操作并發(fā)癥明顯減少,差異有統(tǒng)計(jì)學(xué)意義(P<0.05),見(jiàn)表1。

        3 討論

        鞘內(nèi)置管給藥是動(dòng)物實(shí)驗(yàn)中不可或缺的一項(xiàng)實(shí)驗(yàn)技術(shù),上世紀(jì)70年代,Yaksh等[6]首次提出經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管給藥技術(shù),經(jīng)過(guò)多年的應(yīng)用,這項(xiàng)技術(shù)雖然達(dá)到了鞘內(nèi)置管給藥的目的,但是較高的失敗率、癱瘓率、死亡率嚴(yán)重影響實(shí)驗(yàn)效果。尋找一種安全、有效、并發(fā)癥小的鞘內(nèi)置管技術(shù)是國(guó)內(nèi)外相關(guān)學(xué)者們一直努力的方向。Poon等[9]采用顯微外科技術(shù)在大鼠胸段進(jìn)行鞘內(nèi)置管,得到了較好的成效,不但成功率高,而且還減少了并發(fā)癥的發(fā)生,但是,該項(xiàng)操作技術(shù)復(fù)雜,所用設(shè)備特殊,嚴(yán)重限制了其在動(dòng)物實(shí)驗(yàn)中的廣泛應(yīng)用。近些年,用微量注射器經(jīng)皮穿刺鞘內(nèi)給藥技術(shù)逐漸被大家認(rèn)可[10-11]。這項(xiàng)技術(shù)有創(chuàng)傷小、術(shù)后恢復(fù)快、減少腦脊液外漏、可反復(fù)操作等諸多優(yōu)點(diǎn),尤其在無(wú)脊髓節(jié)段的腰骶部進(jìn)行經(jīng)皮穿刺鞘內(nèi)給藥,不但成功率高,而且安全可靠,不會(huì)對(duì)脊髓造成損傷,但是應(yīng)用在有脊髓節(jié)段的胸段時(shí),操作則無(wú)法進(jìn)行,由于是盲探穿刺,光憑借手下的落空感判斷是否操作成功,極易引起脊髓損傷,最終導(dǎo)致動(dòng)物癱瘓甚至死亡。

        本研究參考上述多種給藥方式的優(yōu)缺點(diǎn),并結(jié)合人蛛網(wǎng)膜下腔麻醉技術(shù)和連續(xù)硬膜外麻醉技術(shù)的操作方法,提出經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù)[12]。從L6~S1間隙進(jìn)針,利用落空感、鼠尾擺動(dòng)、回抽腦脊液等多種方式作為判斷穿刺成功的標(biāo)志,隨后置入導(dǎo)管至腰膨大處,通過(guò)鞘內(nèi)注入局麻藥的方式再次驗(yàn)證鞘內(nèi)置管的成功率。這項(xiàng)操作技術(shù),保留了手術(shù)創(chuàng)傷小、術(shù)后恢復(fù)快、可反復(fù)操作等微量注射器經(jīng)皮穿刺給藥技術(shù)的優(yōu)點(diǎn),避免了其操作上的部位限制;也保留了經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管技術(shù)將藥物直接注入腰膨大部位、節(jié)省用藥量的優(yōu)點(diǎn),同時(shí)減少了其操作創(chuàng)傷大、導(dǎo)管鞘內(nèi)走行節(jié)段長(zhǎng)、易造成腦脊液漏和脊髓損傷等并發(fā)癥的缺點(diǎn)[13-14]。

        本研究表明,與Yaksh法相比,經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù)成功率高,癱瘓、死亡等并發(fā)癥明顯減少,充分證明了經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù)的優(yōu)越性。究其原因:(1)與經(jīng)皮切開(kāi)寰枕膜鞘內(nèi)置管技術(shù)直接暴露脊髓相比,腰骶部穿刺位置較低,不會(huì)接觸脊髓,即使對(duì)馬尾神經(jīng)有少數(shù)損傷也不會(huì)引起癱瘓等嚴(yán)重并發(fā)癥。(2)腰骶部相對(duì)枕骨大孔,離腰膨大部位較近,導(dǎo)管鞘內(nèi)走行距離較短,上行不經(jīng)過(guò)脊髓頸曲與腰曲,發(fā)生脊髓損傷、導(dǎo)管折疊的可能較小。(3)與Yaksh法切開(kāi)直視下置管相比,經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù)屬于微創(chuàng)操作,不用切開(kāi)皮膚、肌層、蛛網(wǎng)膜甚至椎板,對(duì)大鼠損傷較小,炎癥反應(yīng)也相對(duì)較輕,更利于大鼠的術(shù)后存活。(4)經(jīng)皮穿刺腰骶部鞘內(nèi)置管技術(shù)操作簡(jiǎn)單,對(duì)大鼠損傷較小,可以反復(fù)操作,單次給藥,避免了Yaksh法鞘內(nèi)留置導(dǎo)管導(dǎo)致的局部神經(jīng)炎癥反應(yīng)、神經(jīng)變性反應(yīng)、大鼠對(duì)傷害性刺激反應(yīng)改變等副作用[15]。有研究表明,經(jīng)皮穿刺技術(shù)不會(huì)引起大鼠痛閾的改變以及脊髓小膠質(zhì)細(xì)胞和星形膠質(zhì)細(xì)胞的變化,能夠大限度的減少鞘內(nèi)置管操作對(duì)實(shí)驗(yàn)結(jié)果的影響[10]。(5)由于本方法操作簡(jiǎn)單,手術(shù)時(shí)間很短(10 min左右),損傷小,若將腹腔注射麻醉劑戊巴比妥鈉改為異氟醚氣體麻醉或是丙泊酚腹腔注射麻醉[8-10],鞘內(nèi)置管給藥后停留1~2 min即把管抽出,20~30 min后即可檢測(cè)藥物對(duì)大鼠的痛行為的影響,大大提高了實(shí)驗(yàn)效率,且在不同時(shí)間點(diǎn)可多次重復(fù)操作。endprint

        總之,經(jīng)皮穿刺腰骶部置管技術(shù)不但操作簡(jiǎn)單、成功率高,而且并發(fā)癥少,是一項(xiàng)安全有效的鞘內(nèi)給藥新技術(shù)。

        總之,經(jīng)皮穿刺腰骶部置管技術(shù)不但操作簡(jiǎn)單、成功率高,而且并發(fā)癥少,是一項(xiàng)安全有效的鞘內(nèi)給藥新技術(shù)。
