        大鼠脊神經(jīng)選擇性結(jié)扎鏡像痛模型的篩選建立

        【摘要】 目的:研究行為學(xué)觀察和熱輻射縮足反應(yīng)潛伏期(PWTL)及機(jī)械縮足反應(yīng)潛伏期(MWT)實(shí)驗(yàn)篩選建立脊神經(jīng)選擇性結(jié)扎(SNL)鏡像痛大鼠模型。方法:選取100只成年雄性SD大鼠,將其隨機(jī)分為SNL組90只和假手術(shù)組(Sham組)10只,于術(shù)前1 d和術(shù)后1、3、7、14、21、28 d進(jìn)行行為學(xué)觀察和雙足PWTL及MWT實(shí)驗(yàn)檢測(cè)。結(jié)果:兩組術(shù)前行為學(xué)觀察、PWTL及MWT值比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),術(shù)后SNL組手術(shù)足均出現(xiàn)添足、咬足等行為學(xué)現(xiàn)象,且PWTL值和MWT值術(shù)后1~28 d均明顯低于Sham組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。SNL組有14只大鼠為手術(shù)足,于術(shù)后7 d出現(xiàn)添足、咬足等行為學(xué)現(xiàn)象,術(shù)后14、21、28 d的MWT值均明顯低于Sham組,且術(shù)后21、28 d的PWTL值均明顯低于Sham組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論:大鼠SNL鏡像痛模型篩選和建立成功。

        【Abstract】 Objective:To study choice and establishment of mirror-image pain model of selective spinal nerve ligation (SNL) in rats through the behavior observation,paw withdrawal thermal latency(PWTL) and mechanical withdrawal threshold(MWT) test.Method:100 adult male SD rats were randomly divided into the SNL group for 90 rats and the sham operation group(Sham group) for 10 rats.All rats were observed behaviors,detected PWTL and MWT at the 1 day before operation and the 1,3,7,14,21,28 day after operation.Result:There had no significant differences in behavioral observation,PWTL and MWT values between the two groups at the 1 day before operation(P>0.05).Post-operation,SNL groups operated-foot appeared hit feet,bite feet behavioral phenomenon,and the PWTL values and MWT values were significantly lower compared with Sham group at 1-28 d post-operation,the differences were statistically significant(P<0.05).There were 14 rats of the SNL group appeared hit feet,bite feet behavioral phenomenon in 7 d post-operation at un-operation foot,appeared MWT values were significantly lower compared with Sham group at 14,21,28 d post-operation and PWTL values were significantly lower compared with Sham group at 21,28 d post-operation,the differences were statistically significant(P<0.05).Conclusion:The choice and establishment of mirror-image pain model of SNL rats are successful.

        鏡像痛是指當(dāng)軀體某一組織和部位發(fā)生損傷后,導(dǎo)致?lián)p傷側(cè)和對(duì)側(cè)相應(yīng)部位均出現(xiàn)疼痛和痛敏現(xiàn)象[1]。臨床上一些復(fù)雜的區(qū)域疼痛綜合征及慢性病理性疼痛等均可出現(xiàn)鏡像痛[2]。臨床治療效果不佳,動(dòng)物實(shí)驗(yàn)研究較少。目前對(duì)鏡像痛模型的研究多集中在對(duì)坐骨神經(jīng)損傷后建立神經(jīng)病理性疼痛模型的基礎(chǔ)上篩選鏡像痛模型,但在篩選方法、篩選標(biāo)準(zhǔn)、篩選成功率方面鮮見(jiàn)報(bào)道。本實(shí)驗(yàn)在建立坐骨神經(jīng)選擇性結(jié)扎(SNL)神經(jīng)病理性疼痛模型基礎(chǔ)上進(jìn)行篩選以建立鏡像痛模型,為鏡像痛的機(jī)制研究提供實(shí)驗(yàn)依據(jù),現(xiàn)報(bào)告如下。

        1 材料與方法

        1.1 實(shí)驗(yàn)動(dòng)物分組 選取100只成年SD大鼠(漯河醫(yī)學(xué)高等??茖W(xué)校實(shí)驗(yàn)動(dòng)物中心),雄性,體重250~300 g,將其隨機(jī)分為SNL組90只和假手術(shù)組(Sham組)10只,兩組大鼠的一般資料比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性。

        1.2 制作方法

        1.2.1 坐骨神經(jīng)選擇性結(jié)扎模型組(SNL組) SNL組90只,制作方法如下:以氯胺酮10 mg/kg和甲苯噻嗪100 mg/kg腹膜腔注射,大鼠麻醉后取俯臥位,后用聚維酮碘溶液消毒皮膚,于背部脊椎L3~S2節(jié)段作正中切口,分離右側(cè)脊柱旁肌肉群,顯露脊椎并咬斷L4~L6椎骨橫突,仔細(xì)分離并暴露的L4~L6脊神經(jīng),于近脊柱正中處用5-0絲線結(jié)扎L5、L6脊神經(jīng),并于結(jié)扎遠(yuǎn)端進(jìn)行神經(jīng)離斷,后分層縫合切口。endprint

        1.2.2 假手術(shù)組(Sham組) Sham組10只,僅右側(cè)顯露脊椎并咬斷L4~L6椎骨橫突,分離并暴露的L4~L6脊神經(jīng)但不予結(jié)扎,其余方法同SNL組。

        1.3 疼痛行為學(xué)及痛閾測(cè)定 用Von Frey纖毛測(cè)定大鼠的機(jī)械縮足反射閾值(mechanical withdrawal threshold,MWT)以評(píng)價(jià)大鼠機(jī)械痛敏,用熱痛刺激儀測(cè)定大鼠熱刺激縮足反射潛伏期(paw withdrawal thermal latency,PWTL)以評(píng)價(jià)大鼠熱痛敏。每只大鼠均進(jìn)行標(biāo)號(hào)并在手術(shù)前1 d和術(shù)后1、3、7、14、21、28 d分別測(cè)定MWT、PWTL值,每次測(cè)量均先測(cè)定MWT,再測(cè)定PWTL,在測(cè)量前先觀察其添足、咬足、曲足等行為且每次測(cè)量均在上午8:00~10:00之間進(jìn)行。

        1.3.1 MWT測(cè)定方法 采用von Frey絲測(cè)定雙后肢機(jī)械痛閾,將大鼠置于金屬籠中適應(yīng)環(huán)境30 min,用不同壓力纖維絲刺激足底2、3趾間皮膚,每根重復(fù)5次,每次間隔5 s,直至找到出現(xiàn)50%(10次)抬足反應(yīng)的纖維絲,其壓力值(出現(xiàn)大于5次以上是縮足反應(yīng)),即為閾值,左右足輪流測(cè)試,每足測(cè)量5次,取平均值[3]。

        1.3.2 PWTL測(cè)量方法 采用鼠爪熱輻射刺激測(cè)痛儀[3]。先讓大鼠處于自由活動(dòng)狀態(tài),調(diào)整好刺激強(qiáng)度(r=72),將刺激裝置放置在鼠爪下,按操作鍵,當(dāng)鼠爪移動(dòng)時(shí)系統(tǒng)自動(dòng)記錄大鼠反應(yīng)時(shí)間,此時(shí)間即為大鼠足底PWTL,每足測(cè)量5次,時(shí)間間隔3~5 min,取平均值。

        1.4 SNL鏡像痛大鼠的篩選 實(shí)驗(yàn)結(jié)束后,對(duì)SNL組大鼠的實(shí)驗(yàn)記錄進(jìn)行分析,凡術(shù)后雙足均出現(xiàn)行為學(xué)異常、PWTL值和MWT值降低的大鼠標(biāo)記為SNL鏡像痛組大鼠,其余為SNL非鏡像痛組。

        1.5 統(tǒng)計(jì)學(xué)處理 采用SPSS 13.0軟件對(duì)所得數(shù)據(jù)進(jìn)行統(tǒng)計(jì)分析,計(jì)量資料用(x±s)表示,比較采用t檢驗(yàn),以P<0.05為差異有統(tǒng)計(jì)學(xué)意義。

        2 結(jié)果

        2.1 行為學(xué)觀察 SNL組與Sham組術(shù)前無(wú)特異性行為,兩組大鼠術(shù)后均出現(xiàn)舔舐傷口行為,SNL組大鼠行坐骨神經(jīng)選擇性結(jié)扎術(shù)后7 d內(nèi)手術(shù)側(cè)出現(xiàn)添足、咬足、垂足、腳趾并攏、屈曲等行為;SNL組7 d內(nèi)對(duì)側(cè)足也出現(xiàn)上述行為的定義為SNL鏡像痛組大鼠,共篩查出14只,其余為SNL非鏡像痛大鼠,共76只,對(duì)側(cè)足未出現(xiàn)上述行為。

        2.2 各組大鼠PWTL和MWT值的比較 各組術(shù)前PWTL和MWT值比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),術(shù)后1~28 d,SNL鏡像痛組和SNL非鏡像痛組手術(shù)足PWTL和MWT值均明顯低于Sham組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05)。SNL鏡像痛組對(duì)側(cè)足MWT值于術(shù)后14、21、28 d均明顯低于Sham組,且PWTL值于術(shù)后21、28 d均明顯低于Sham組,差異均有統(tǒng)計(jì)學(xué)意義(P<0.05);而SNL非鏡像痛組與Sham組的對(duì)側(cè)足術(shù)后PWTL和MWT值比較差異均無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),見(jiàn)表1~2。

        3 討論

        鏡像痛在臨床和動(dòng)物實(shí)驗(yàn)中均較為普遍,表現(xiàn)為對(duì)側(cè)肢體相應(yīng)部位出現(xiàn)與患側(cè)肢體相同的持續(xù)性疼痛、局限性疼痛、灼燒性痛、感覺(jué)異常、出汗等,如臨床上復(fù)雜的區(qū)域疼痛綜合征、截肢和中風(fēng)后遺癥等,加重患者負(fù)擔(dān),給患者帶來(lái)極大痛苦[2]。

        對(duì)于鏡像痛的動(dòng)物實(shí)驗(yàn)研究較早見(jiàn)于Zhang等[4]在2000年的單節(jié)段背根神經(jīng)節(jié)慢性壓迫實(shí)驗(yàn)中發(fā)現(xiàn)了鏡像痛現(xiàn)象的報(bào)道。后關(guān)于鏡像痛的報(bào)道陸續(xù)出現(xiàn),Arguis等[5]發(fā)現(xiàn)在L5、L6脊神經(jīng)結(jié)扎模型中,大鼠同側(cè)和對(duì)側(cè)后肢均出現(xiàn)機(jī)械刺激和冷刺激的痛覺(jué)異常。Schreiber等[6]在小鼠后足注射急性致炎物質(zhì)角萊膠后,發(fā)現(xiàn)對(duì)側(cè)后足出現(xiàn)了短暫延遲隨后穩(wěn)定持久的疼痛現(xiàn)象。

        鏡像痛模型的構(gòu)建方面,周圍神經(jīng)損傷引起鏡像痛的報(bào)道較多,如關(guān)節(jié)炎模型、癌癥痛模型,但脊神經(jīng)損傷模型最為多見(jiàn),常見(jiàn)的脊神經(jīng)損傷模型有坐骨神經(jīng)慢性壓迫模型(CCI)、脊神經(jīng)選擇性結(jié)扎模型(CNL)、脊神經(jīng)選擇性損傷模型(SNI)及脊神經(jīng)背根神經(jīng)節(jié)慢性壓迫模型(CCD)等[7-12]。但在疼痛模型基礎(chǔ)上如何篩選鏡像痛模型的篩選方法、篩選標(biāo)準(zhǔn)、篩選成功率方面鮮見(jiàn)報(bào)道。

        本實(shí)驗(yàn)在成功建立坐骨神經(jīng)選擇性結(jié)扎(SNL)神經(jīng)病理性疼痛模型基礎(chǔ)上,通過(guò)行為學(xué)觀察和MWT及PWTL值進(jìn)行篩選,以SNL組實(shí)驗(yàn)全程出現(xiàn)非手術(shù)側(cè)足有上述特異性表現(xiàn)并與Sham組比較有統(tǒng)計(jì)學(xué)意義為依據(jù)建立SNL鏡像痛模型。研究發(fā)現(xiàn),SNL鏡像痛模型組手術(shù)側(cè)和非手術(shù)側(cè)均出現(xiàn)添足、咬足等行為學(xué)現(xiàn)象和MWT及PWTL值降低,與Sham組相比機(jī)械刺激在術(shù)后14 d時(shí)出現(xiàn)明顯差異(P<0.05),而熱刺激在術(shù)后21 d時(shí)才出現(xiàn)明顯差異(P<0.05),說(shuō)明在SNL鏡像痛模型中機(jī)械刺激較熱刺激痛覺(jué)異常出現(xiàn)較早并發(fā)生了時(shí)間延遲。本實(shí)驗(yàn)使用90只成年雄性SD大鼠進(jìn)行SNL造模,共獲得14只SNL鏡像痛大鼠模型,但由于樣本數(shù)量較少,篩選成功率僅供研究者參考。

        總之,本實(shí)驗(yàn)在成功建立大鼠SNL神經(jīng)病理性疼痛模型的基礎(chǔ)上通過(guò)行為學(xué)觀察和MWT及PWTL值篩選鏡像痛模型成功,為鏡像痛的機(jī)制研究提供可靠的實(shí)驗(yàn)依據(jù)。
